Health Management Services
Through Health Management Services, Renown Health offers a number of educational and support programs for patients. These programs help people overcome the challenges presented through health conditions such as asthma or diabetes and to aid people in creating and adopting a healthy lifestyle.
The Health Management Services team of Registered Dieticians, Respiratory and Physical Therapists and Physicians works with you and your loved ones to provide the tools to manage your disease and live a happy and healthy life.
Diabetes Programs
- Diabetes Clinic (physician managed) - Comprehensive disease management program for people with both Type 1 and Type 2 diabetes.
- Type 1: Self-Management Program - a 3-visit program for people with Type 1 diabetes regardless of duration.
- Type 2: Self-Management Program (physician managed upon request) - Comprehensive training for those with Type 2 diabetes. Designed to make the individual an "expert" in their diabetes. Best program for those taking insulin or having had diabetes greater than two years.
- Type 2: Survival Skills Training - A 3-visit program designed for those people with newly diagnosed diabetes, impaired glucose tolerance (pre-diabetes) or those having no previous diabetes education.
- Gestational Diabetes Training - Training in self-blood glucose monitoring, ketone testing, use of results and insulin administration (as needed). Comprehensive meal planning and nutritional information for women with gestational diabetes.
