Catholic Charities of Northern Nevada

Catholic Charities of Northern Nevada is committed to providing wrap-around support services to at-risk populations, such as low-income children, families and seniors citizens. Through the KSK program, social workers, outreach specialists and registered nurses conduct outreach services in targeted neighborhoods to identify and serve individuals who need support and referrals to community agencies.

Community Health Alliance — Nevada

Community Health Alliance is a non-profit federally qualified health center formed in July 2012 through the merger of Health Access Washoe County (HAWC) and Saint Mary’s Mission Outreach programs. CHA is the largest provider of primary medical and dental services to Medicaid patients in Northern Nevada, and serves many patients without insurance. CHA operates six primary care health centers in Washoe County, as well as mobile preventive and restorative dental programs, and the Women, Infants and Children (WIC) nutrition program.

HOPES provides integrated medical and wellness services, with a focus on lowering barriers to care for underserved communities. Services offered include adult and pediatric primary care, chronic disease management, behavioral health counseling, substance use counseling, case management, nutrition, pharmacy, transportation, and community outreach and education. Urban Roots is an agricultural-based education nonprofit dedicated to growing healthy minds, bodies, and communities. Urban Roots helps children answer the question, “where does food come from?” and strives to change the way children eat and learn. Through school-based lessons, hosting children at their teaching farms, and offering programming to use the garden as an outdoor classroom, Urban Roots brings seed-to-table education to the northern Nevada community, its children, and the educators who teach them.
