The incumbent is expected to provide optimal patient care by recognizing differences in drug therapy (i.e drug, dose & frequency) in pediatric, adolescent, adult, and geriatric patients.
Incumbent is challenged to assist the physician, nurse and pharmacist in the timely and accurate development of an accurate home medication list. Questions involving medication use or therapeutics must be referred to the pharmacist and good judgment is required when other types of information is provided by the technician.
The incumbent is responsible for interviewing the patient and utilizing other resources available (i.e. outpatient pharmacies, family, etc) to obtain a list of homemedications, over the counter products and supplements that the patient is currently taking. The incumbent will be responsible for entering medication orders into the pharmacy computer system; this involves maintaining and updating the patient medication reconciliation profile; all therapeutic questions are to be referred to the pharmacist. The incumbent is expected to act in a professional manner and to provide exceptional patient service as directed by the Renown Service fundamental.
The incumbent is challenged to assist in the training of new technicians, pharmacy students, pharmacy practice residents and nursing staff in medication reconciliation.
The incumbent will be required to work varying shifts and days to meet the needs of the department which operates 24 hours a day, 7 days a week (Cost Center 200 only). Overtime may be required. The incumbent is expected to attend on a regular basis staff meetings, technician meetings, etc.
In addition to being competent to work in the inpatient & outpatient environments, incumbent is expected to be trained and have the ability to work in multiple areas. This position must maintain general pharmacy knowledge, knowledge of medications, medication dosage forms, pharmaceutical calculations and trade and generic names. Thorough understanding of medication order procedures involving the pharmacy computer system. Demonstrates interpersonal skills to maintain effective communication with co-workers, other hospital employees, patients and visitors. Working knowledge of Renown Health and specific departmental policies and procedures, as well as state and federal laws regarding pharmacy.
The incumbent may perform other duties as assigned.
