In case management we use a unit-based case management model and work across all departments within the hospital. We handle utilization review using the InterQual Criteria. Our staff of registered nurse case managers works closely with social work in the discharge planning process and has created a strong sense of teamwork among all staff members and providers. The entire collaborative team shares the commitment to find the best ways to serve our patients using our resources wisely.
But the Case Management Department is not just about the case managers themselves. We are fortunate to have additional staff called transfer coordinators who provide clerical support with referrals by using our Web-based referral system, the Extended Care Information Network (ECIN). They handle the clerical duties involved in getting referrals, which allows the case managers to be more efficient and focus on using their nursing skills to better manage the care of our patients.
As our hospital grows, so does our department. There is tremendous opportunity for any nurse who wants to be part of the exciting transformation that is taking place in our organization today.
The Case Management Department at our newest 155-bed hospital includes a combination of social workers and nurses. It has proven to be highly effective and is working especially well in the smaller hospital setting.
