Helping Patients Achieve Independence
Renown Rehabilitation Hospital provides 24-hour nursing care with nurse/therapist-to-patient ratios that are unmatched in the region. Case managers are assigned to assist in determining the best plan of care from admission to discharge. Patients receive a minimum of three hours of therapy every day, and a minimum of 15 hours per week, based on their functional abilities. We offer intensive therapy services to guide patients to higher function, independence and wellness.

Follow -Up Care and Physiatry Services
Renown Medical Group - Physical Medicine & Rehabilitation provides follow-up care for patients. This program improves function and increases independence and features:

- Specialty Physiatrist follow up care for spinal cord injury, brain injury, stroke, and amputations

Team Approach to Care
Nurses provide ongoing assessments of patients' needs, coordination and implementation of care, re-evaluation of interventions and education on many topics, including medications and managing healthcare needs after leaving the hospital.
Your rehabilitation nurse will also work with the rehabilitation team to reinforce mobility goals outside of therapy sessions.
Your team includes an attending physician and nurse practitioner who are experts in rehabilitation medicine. Your team also includes other specialists such as a Renown Hospitalist if necessary.
A nurse practitioner will also work in collaboration with your physician to provide general medical management, including assessment, diagnosis and treatment.
At the time of discharge, you will be referred back to your primary care physician and other physicians as indicated.
We treat certain medical conditions with dietitian-supervised meal plans. The goal is to reduce complications in pre-existing conditions and alleviate the effects of any existing condition.
The role of the dietitian is to ensure proper nutrition and provide diet education for your health and well-being.
Taking medications might help you manage pain, feel better, control disease or prevent complications. Your pharmacist will work with you to ensure optimal health.
Weekly classes are available where you can speak to your pharmacist about medication management and any concerns you might have.
Your case manager coordinates your care by working with you and your family regarding discharge plans from Renown Rehabilitation Hospital. The case manager also serves as the liaison between your family, physician, treatment team, insurance carrier and community agencies.
Upon discharge, you will be referred back to your primary care physician for your medical needs and your case manager will arrange additional physician referrals. A copy of your discharge summary will be sent to your primary care physician.
Once you are discharged, you will receive a follow-up phone call from the Case Management Department to see how you are doing with your therapy, medications and home care. They can also answer any questions you may have.
