Responsible for building collaborative relationships with Nurse Managers, Associate Nurse Managers, Nursing Supervisors, Charge RN’s, ancillary leaders, and administration. Develops and implements plans to identify and mitigate impediments to efficient patient flow throughout the hospital system. Utilizes clinical skills, critical thinking, and collaborative decision making with others to increase patient flow.
Works proactively with nursing units to identify internal barriers to discharge. Works closely with the leadership of ancillary departments in the escalation of testing and corresponding resulting as well as procedure completion. Provides follow-up confirmation to nursing unit leadership/charge RN once barriers to discharge have been removed and/or completed.
Utilizes tools within the electronic medical record (EMR) to identify patients nearing readiness for discharge and coordinates the completion of tasks with the clinical team. In collaboration with Renown Transfer and Operations Center (RTOC), proactively monitors daily flow within the hospital to identify and mitigate bottlenecks in patient flow.
Monitors patients with active transfer back agreements for readiness to transfer back to sending facility and communicates this information to the clinical team for initiation.
Attends daily Patient Safety and Capacity huddle and Patient Flow Committee meetings. Attends Interdisciplinary Team rounds on nursing units weekly.
Educates providers, staff, and Charge RNs on global patient flow issues in order to maximize the use of EMR based discharge readiness tools.
Collaborates with leadership to quarterly assess and improve flow plans.
Performs other duties and projects as assigned.
