Today, in accordance with recommendations from the U.S. Surgeon General and the American College of Surgeons, the Renown Health Joint Medical Executive Committee decided that effective Tuesday, March 17, surgical cases scheduled at both Renown Regional Medical Center and Renown South Meadows Medical Center, considered to be ELECTIVE-OPTIONAL will be postponed to a later date.  Patients with scheduled ELECTIVE-OPTIONAL surgeries will be called by Renown Health representatives over the next week. Surgeon’s offices will also be reaching out to assist patients with rescheduling their surgeries for a future date.  Elective surgeries that are REQUIRED and not optional will be evaluated on a case-by-case basis daily with the surgeon, patient and a medical leadership review committee at Renown. Your surgeon’s office will be in contact with you with more information about these REQUIRED surgeries This temporary measure will allow Renown’s physicians and staff to enact the emergency preparedness plans they have been developing for weeks to create additional capacity for inpatients and to continue to deliver high quality care during the anticipated surge in COVID-19 cases in northern Nevada. The new guidelines do NOT apply to EMERGENCY surgeries performed in Renown hospitals. There are different types of surgery, this decision impacts Elective-Optional surgeries only. Elective – Optional surgery – Elective surgeries are not usually necessary for the individual to stay in good health. They are non-emergency and planned in advance. A wide range of surgeries can be elective. Most cosmetic surgeries are elective. Other surgeries such as organ donation, scoliosis surgery, tonsillectomies and other minor surgeries can also be considered elective. Required surgery – Surgery which needs to be done in order to retain quality of life. As opposed to urgent or emergency surgery, required surgeries do not need to be performed immediately. Examples of required surgery are kidney stone or tumor removal. Urgent or emergency surgery – When a patient’s condition is life threatening, surgery is considered emergent. Emergency surgeries must be performed immediately, even when the patient is unconscious and cannot give consent. Examples include trauma and appendicitis. Life-threatening conditions can occur during labor and delivery which may require emergency surgery. Other services making changes include: Pulmonary Services and Procedures Renown Health today will begin rescheduling patients scheduled for Pulmonary Function Testing, Outpatient bronchoscopy and patients at Thomas S. Dolan Pulmonary Rehabilitation. Patients who are affected by this change are being called by Renown to reschedule their appointments for Monday, April20, 2020 or later. Starbucks at Renown and Renown Dining Options As with many of the Starbucks around the U.S., the Starbucks at Renown Regional Medical Center will shift to a “to-go” model, with no seating available. The cafeterias at Renown Regional Medical Center and South Meadows Medical Center remain open, will temporarily eliminate self-service options and will add more ‘grab and go’ options. Other Renown Services Remain Open Renown Medical Group offices remain open. Expanded test processing capabilities is resulting in faster test results, lessening the load on the state lab, and helping keep patients, care providers and the community safe. Renown Health announced today that its in-house laboratory is now able to process up to 500 COVID-19 tests for hospital patients and care providers each day. The organization has invested in expanded test processing capabilities to better serve the community amid the coronavirus pandemic, and to increase testing available across the state. Next month, Renown anticipates continuing to further expand testing capacity and doubling its capacity to be able to provide 1,000 tests per day in preparation for the continued response to the pandemic. “The COVID-19 pandemic is not going away- it is still with us. I am concerned by the number of new cases and hospitalizations we are seeing across northern Nevada. We must do more testing. For patients receiving hospital care, these tests help determine if patients should be placed in a designated area of the hospital to mitigate the risk of transmission to those not infected. Furthermore, all surgery patients are tested ahead of their surgery and self-quarantine prior to their surgical procedure at Renown Regional Medical Center and Renown South Meadows Medical Center. How do PCR tests work? PCR tests use a sample of mucus typically taken from a person's nose or throat. The test looks for the genetic material of the coronavirus, using a technology called PCR to detect if the viral genetic material is present. Those actively infected with the virus will show the existence of that material. Together, will test over 30,000 qualifying study participants by 2023 for risk of cirrhosis and liver-related illnesses. Renown Institute for Health Innovation (IHI) announced today that the organization and Gilead Sciences, Inc. will be joining forces with Siemens Healthineers to offer the Enhanced Liver Fibrosis (ELF™) Test to people with risks for nonalcoholic steatohepatitis (NASH) enrolled in the Healthy Nevada Project (HNP). The ELF Test will help identify people most at risk for progressing to cirrhosis and liver-related outcomes and allow healthcare providers to intervene before irreparable damage occurs. This noninvasive blood test uses three serum biomarkers to create an ELF score from a predefined algorithm, which can be used by doctors to help evaluate if a patient requires increased medical care and monitoring for their condition. Nonalcoholic fatty liver disease (NAFLD), which includes NASH, is prevalent in Nevada and under-diagnosed, likely affecting more than 500,000 adult Nevadans. If undetected and untreated, NASH can result in liver cirrhosis and may require liver transplantation or lead to death. It is normal for the liver to contain some fat, but if more than 5 percent of the liver content is fat, it’s considered a fatty liver (steatosis). NASH is the most severe form of NAFLD in which a person has liver cell damage and inflammation of the liver. Inflammation and liver cell damage can cause fibrosis, or scarring of the liver, and can cause decreased liver function (1). The symptoms of NASH are often silent or non-specific, making it difficult to diagnose. About one-third of people with NASH develop cirrhosis or irreversible liver damage (2). About the ELF™ Test The ELF Test is a noninvasive blood test that can quickly identify which patients are at an elevated risk for developing cirrhosis and other liver-related clinical events (LREs). In contrast to standard liver enzyme tests that reflect liver damage that has already occurred, the ELF Test combines three serum direct biomarkers of active fibrosis. The ELF Test algorithm measures each of these biomarkers to create an ELF score, which can be used as an aid to assess the risk for future disease progression. Doctors may use this ELF score to help evaluate if a patient requires increased medical care and monitoring for their condition. Individuals interested in determining their risk for NASH and its progression are encouraged to enroll in the Nonalcoholic Steatohepatitis Liver Disease Genome Atlas study. Renown joins Washoe County Health District to Offer COVID-19 Vaccines to Children 5-11 at Vaccine Clinics. Renown Health is partnering with the Washoe County Health District to help distribute the initially available, limited supplies of an age-appropriate dose of the COVID-19 Pfizer-BioNTech vaccines to children ages 5-11. At this time, all residents 5 years and older are eligible to receive a vaccine. Renown announced today that for parents/guardians with proxy access for their children through the Renown MyChart app, vaccination appointments are now available for children ages 5-11. Pending supply, Renown expects to be able to vaccinate 1,600 children with the 2-dose series in a limited series of weekend clinics. At this time, due to limited supplies, no vaccines for children are available through Renown Medical Group pediatric offices. Beginning Thursday, January 14, 2021, in order to quickly vaccinate additional residents of Washoe County age 75 and older, Renown Health is pleased to partner with the Washoe County Health District to help distribute the initially available, limited supplies of COVID-19 vaccines.  Those who are 75 years or older interested in joining the waiting list for an appointment at Renown’s Drive Thru COVID19 vaccination clinic, will need to download Renown MyChart, if they currently do not have an account. Residents do not need to have a Renown Health provider to enroll. Residents will be notified through their MyChart account when appointments are available. The vaccine clinic is operating by appointment only. Due to limited resources and the limited vaccine available, initially the Renown COVID-19 Vaccination Program will concentrate on vaccinating those 75 and older at this time in Washoe County. Per Governor Sisolak, and the state’s vaccine rollout plan, once those 75 and older have the opportunity to receive the vaccine, the next general population priority group is those who are 70 and older. In addition, Renown Health patients and members of Senior Care Plus/Hometown Health aged 75 and older are will be notified through their Renown MyChart accounts as appointments become available. The greatest risk for severe illness from COVID-19 is among those aged 85 or older. Severe illness means that a person with COVID-19 may require hospitalization, intensive care, or a ventilator to help them breathe, or they may even die. In Washoe County alone, there are 29,034 people 75 years or older and Renown is only receiving limited supplies of the vaccine. Older adults are at greater risk of requiring hospitalization or dying if diagnosed with COVID-19. As you get older, your risk of being hospitalized for COVID-19 increases. For those aged 75 and older, the risk of hospitalization is eight to 13x higher, with deaths 220-630 percent higher than for younger adults. Due to the unprecedented demand of local residents, Renown now has a full “waiting list” for those interested in the COVID-19 vaccine, and will be working through the list to offer appointments as they become available.  Renown continues to vaccinate physicians and its essential workforce for COVID-19, and to provide nasal swab testing for those with a lab order from their provider, for COVID-19 symptoms. In addition, Renown has stretched to support the Washoe County Health District to meet the needs of those aged 70 and older who are now eligible to receive a COVID-19 vaccine.  Over this past weekend, more than 3,000 of Washoe County’s oldest residents were able to obtain an appointment, and receive their first dose of the COVID-19 vaccine at Renown’s Drive Thru COVID-19 vaccination clinic. Renown posted 3,000 appointments for those on the MyChart electronic waiting list and 100% of appointments were booked within an hour. “We want to help the Health District, and we are working to vaccinate as many people as possible, given our limited staffing, volunteers, space and vaccine,” says Melodie Osborn, RN, Chief Nursing & Quality Officer for Health Services. “We are glad to step in and help; and our goal is to always provide an exceptional vaccine experience. Our team and our systems are definitely feeling the pressure as we see unprecedented demand for this vaccine. We ask the public to be patient with us, and to help support us in this important work.” Scheduling limited Both the Pfizer and Moderna COVID-19 vaccines require two doses, the second administered three to four weeks after the first. Renown is working to be sure those who receive their first dose also obtain their second dose in a timely manner. This includes 7,000 physicians and health care workers, and will now include those older adults vaccinated with their first dose. The complexity of the vaccine dosage requirements continues to draw down on the number of vaccines and appointments available.  Renown MyChart and phone systems strained under the demand Last Thursday, when Renown announced that those who are 75 years or older could join the waiting list by downloading Renown Epic MyChart, so many people logged in that the system was strained to accommodate the additional demand. Renown has now doubled the capacity of the Epic MyChart system to be sure all goes smoothly.  Renown Health has been able to distribute and administer the COVID-19 vaccine to a record number of people in the shortest possible time. Much of this success is due to the use of Epic MyChart, a convenient patient electronic medical record which allows people to schedule their appointments online, complete the consent form and have full documentation for the future. Also last Thursday, older adults without cell phones, computers or someone to assist them, called Renown’s customer service phone line for assistance. So many people called the phone line that it crashed.
