6 Healthcare Action Items for the LGBTQIA+ Community
Every patient, regardless of how they may identify, greatly benefits from preventive healthcare and early detection. Members of the LGBTQIA+ community face unique considerations when it comes to their health, and a proactive approach to preventive screenings and vaccines is important in order to address their individual health needs. Dr. Karen Thiele, Family Medicine Physician with University Health and Assistant Professor of Family and Community Medicine at the University of Nevada, Reno School of Medicine, breaks down key steps that LGBTQIA+ patients should take to safeguard their health. PrEP and PEP Pre-exposure prophylaxis (PrEP) is a strategy to prevent human immunodeficiency virus (HIV) infection. It is an important measure for those who are HIV-negative but may be at risk of contracting it. The highest risk sexual practice is receptive anal intercourse, due to the relative fragility of rectal tissue. This medication can stop HIV from spreading in the body and help patients maintain their HIV-negative status. PrEP is available in both pill form, which is taken every day, and injection form, of which the first two injections are initiated one month after another while all other injections are initiated every two months. Post-exposure prophylaxis (PEP) is an antiretroviral drug regimen taken after potential HIV exposure to prevent an HIV-negative individual from converting to HIV-positive status. PEP is only for emergency situations and must be started within 72 hours of exposure – sooner is always better than later – and must be taken for 28 days. PrEP and PEP are available in many ways, including visiting your primary care provider (PCP) or an urgent care location. HPV Immunization All genders and identities can protect themselves against human papillomavirus (HPV), a sexually transmitted infection (STI) that can lead to the risk of cervical, mouth, head, neck, throat, anal, vaginal, penile and vulvar cancers. HPV is so common that nearly all sexually active people, regardless of sexual orientation and practices, will be exposed at some point in their lifetime. The HPV vaccine (common brands include Gardasil and Cervarix) is a safe and effective method to prevent HPV, according to the Centers for Disease Control and Prevention (CDC). This vaccine protects against infections that can lead to HPV-related cancers and precancers, as well as genital warts. While patients should start receiving the vaccine at 9 years old years old, unvaccinated adults up to the age of 45 can also receive the vaccine through their PCP – better late than never!

Prevention Against STIs Matters
According to the U.S. Department of Health and Human Services, there are more than 20 million estimated new sexually transmitted infection (STI) cases in the United States each year, with rates continuing to increase. What you may not know is most STIs are preventable. We talked with Renown Adolescent Medicine Specialist, Caroline Barangan, MD to learn more about STIs. How Can You Get an STI? The CDC (Center for Disease Control) says that STIs are acquired through sexual contact. There are bacteria, viruses or parasites that can cause an STI which may pass from person to person in blood, semen, vaginal and other bodily fluids. How Do You Know if You Have an STI? STIs can have a range of signs and symptoms such as: Warts, bumps or sores on or near the penis, vagina, mouth or anus Swelling, redness or severe itching near the penis or vagina Discharge from the penis Vaginal bleeding that’s not your period Painful or uncomfortable sex Vaginal discharge that has an unpleasant odor, causes irritation or is a different color or amount than usual Weight loss, diarrhea or night sweats Aches, pains, fever and chills Jaundice (yellowing of the skin and whites of the eyes) Painful or frequent urination Sore throat if you engage in oral sex It’s important to know that the majority of people who have an STI commonly have no symptoms at all, which is why it’s important to get regularly tested once you have had any sexual activity. Young people less than 25 years of age should be screened on a yearly basis at minimum.

Leading U.S. Hospitals Team Up To Promote COVID-19 Vaccine
A coalition of 60 top hospitals and healthcare institutions have joined forces in a nationwide campaign to encourage adults to get vaccinated against COVID-19. Led by Cleveland Clinic and Mayo Clinic, the campaign "Get the Vaccine to Save Lives" is designed to reassure the public that vaccines are safe, effective and necessary to achieve herd immunity and return to normal activities. "We're asking people to talk to their healthcare providers if they have questions and then get vaccinated," says Gianrico Farrugia, M.D., Mayo Clinic's president and CEO. "The vaccine is our strongest asset to end the pandemic, and I urge everyone who is eligible to get whichever vaccine you're first offered to save lives." The campaign hopes to reach adults who are hesitant to receive a vaccine, including racial and minority ethnic groups and people living in rural communities. According to a Kaiser Family Foundation survey published March 30, 17% of the public say they will take a wait-and-see approach before getting it themselves, and another 20% say they will never get a vaccine or will only get it if required to do so for work, school or other activities. This leaves a significant portion of the population at risk of going unvaccinated. The nationwide campaign includes print and digital advertising, media outreach, social media, an awareness video and an informational website. "The COVID-19 vaccine is safe and highly effective and offers our best hope for beating the disease," said Tom Mihaljevic, M.D., Cleveland Clinic's CEO and president. "We all want to see a return to our pre-COVID routines, when we can once again gather safely with family and friends. To reach that goal, we must improve vaccination rates to achieve herd immunity. Please sign up today because we are all in this together." With vaccine distribution underway, the campaign aims to help Americans feel safe and confident about receiving a vaccine. To achieve herd immunity and help end the pandemic, leading health officials say at least 75% of the population needs to receive a vaccine. Herd immunity occurs when a large portion of a community becomes immune to a disease, making the spread of disease from person to person unlikely. As a result, the whole community becomes protected — not just those who have been vaccinated. Take Charge of Your Sexual Health
Much like general physical and mental health, sexual health is an essential aspect of your overall well-being. Therefore, educating yourself on your sexual health is an important piece regarding your overall wellness. Secure & Private Virtual Visits Did you know that many sexually transmitted disease appointments can happen virtually, in the privacy of your own home? Our providers can perform a screening examination for sexually transmitted illness (without symptoms) when it’s convenient for you. This is a great option if the topic makes you uncomfortable during an in-person visit, or if you can’t find time to visit your primary care provider or local urgent care. After the telehealth appointment, your provider will determine the next steps for you. Remember, in-person visits are always an option as well. Here’s how you can take charge of your sexual health: Get Comfortable with Getting Tested Surprisingly, some people with STDs don't show symptoms, meaning they may not even know they have an STD unless they get tested. Even if an STD shows no symptoms, the result of non-treatment can be serious. Bret Michael Bellard, MD, with Renown Medical Group, shares that it’s important for people to get tested for STDs because if left untreated, they can cause the following health problems: Loss of fertility Pregnancy complications Other health issues When to Visit Your Primary Care Provider Who should get tested and how often? “The recommendations are that everyone from ages 13 to 64 should get tested at least once for HIV. All sexually active adults should also be tested for other STDs once a year, and all pregnant women should be tested at the start of their pregnancy,” says Dr. Bellard. Some STDs, like syphilis, can be passed from mother to baby, so it's important for women to get tested as soon as they know they’re pregnant. Dr. Bellard recommends going to your primary care provider for testing. “They can also give advice on prevention and other health topics.” If you don’t have a primary care provider, this is a great reason to get established with one. Women with an established OB/GYN can have STD testing done simultaneously with their annual pelvic exam. Practicing safe sex and getting tested routinely are your best defenses against all types of STDs. For many, the conversation about sexually transmitted diseases is awkward and many avoid it. To lower your risk of contracting an STD, don’t wait until you’re “in the moment” to have the conversation with your partner about the last time you were both last tested. If you haven’t gotten tested in-between new partners, use latex condoms every time you have sex until you are both confirmed negative of STDs. Condoms are not 100% effective at preventing disease or pregnancy. However, they are extremely effective if used properly. First COVID-19 Vaccines Administered to Physicians Today
This week, many of the nation’s health care systems in communities across the U.S. began vaccinating physicians and health care workers caring for patients with COVID-19. Many of these health care heroes have been on the front-line of this pandemic and working around-the clock over the last ten months. Vaccinations signal hope for the future Here in Washoe County, today is the highly anticipated start date of what public health and medical experts hope is a nationwide wave of vaccinations that signal the beginning of the end of the COVID-19 pandemic. Today’s good news from Renown coincided with a dark new milestone for Washoe County—384 confirmed COVID-19 deaths, and over 13,579 active cases in our population of nearly 500,000. COVID-19 positivity test results and hospitalizations continue to be high across the country, our state and here in northern Nevada; straining hospital capacity, physicians and healthcare workers. Today we saw a bright light at the end of this dark tunnel. Renown began vaccinating health care workers caring for patients with COVID-19, and invited media to view the first vaccinations of those on the front-line of COVID-19. One vaccine, a Herculean effort of thousands “Although the vaccination process as you see it here looks relatively simple, it took a herculean logistical effort to implement the COVID-19 vaccination process that has begun today,” said Tony Slonim, MD, DrPH, President & CEO of Renown Health. “Today, thanks to the synchronized efforts of thousands of people and organizations across the country, we give thanks for this vaccination provided to Luis Martinez, an excellent ambassador for the thousands of healthcare heroes caring for patients with COVID-19. The health and safety of our staff is our top priority, and we remain steadfast in our determination to fight the good fight for our community.” Luis, on the front-line of the COVID-19 pandemic, receives 1st vaccine from Renown Luis Martinez received the 1st of his Pfizer-BioNTech vaccine which received emergency use authorization from the Food and Drug Administration last week. He will receive the second and final vaccination in the series in 21 days. The first dose of vaccine was given to Luis outside of his vehicle so cameras could capture the experience. Renown’s COVID-19 Vaccination process includes pre-scheduling physicians and employees through the Epic MyChart electronic medical record used by Renown. Employees are masked, and in their personal vehicles, as they drive through the vaccination site. Each physician and healthcare worker provides their verification, identification, documentation and consent. They then extend their upper arm to receive the Pfizer vaccination. Following the receipt of the vaccination they have a 15-minute observation period to assure they do not experience any immediate side effects. These Tier 1A healthcare employees were encouraged to schedule their vaccination after their completed work schedule to have time to rest. As with all vaccinations, there is a potential for injection site reactions (redness, swelling and pain) as well as fever, fatigue, headache, chills, vomiting, diarrhea, muscle pain and/or joint pain. Fever is not an infrequent side effect, especially on the 2nd dose, so these vaccinated employees will self-monitor their temperatures, as they cannot work at Renown with a fever. In addition to Luis, you’ll see, first in his truck- Dr. Bret Frey, representing the Medical Staff and Emergency Medicine physicians at Renown; Nurse Matt Polak, Supervisor of Clinical Nursing of Renown’s Respiratory Intensive Care Unit representing Renown nurses; Holly Irwin, a Clinical Respiratory Therapist in her vehicle; Alexis Bertauche, Respiratory Specialist in his vehicle; Eric Whitney, the supervisor representing the 168 members of Renown’s Environmental Services team who have performed extensive disinfecting measures throughout the pandemic, physician representatives from partners at UNR School of Medicine, Reno Orthopedic Clinic, the Surgery Center of Reno and across the community. Brighter days ahead for Luis Luis has been working in Renown’s Alternate Care Site in the Mill Street parking structure since it opened to patients in mid-November. He’s been at Renown since 2016, where he started as a phlebotomist and was encouraged to explore career advancement in the medical field. He applied to work as a technician on Renown’s Clinical Decision Unit- a short stay unit within the hospital for people who need clinical care, but do not need to be admitted to the hospital. Luis worked with patients and clinicians in the Clinical Decision Unit for a year before requesting to work in Renown’s innovative Alternate Care Site, built for patients recovering from COVID-19, for additional care before they returned home. Luis was scheduled to take the Medical College Admission Test® in April of this year to fulfill his dreams of becoming a doctor, but the COVID-19 pandemic forced him to change his plans. He plans to reschedule his medical school admission exam in February. He is excited to begin his training, possibly at UNR School of Medicine, and hopes one day to be a physician caring competently and compassionately for those in need. Luis has so much to look forward to, after such a difficult year. Luis turned 33 in October, and on his birthday, contracted COVID-19 after attending a family party. He experienced fever and noticed how dramatically the COVID-19 virus affected his breathing, his lungs, and his energy level. After spending time at home recovering, Luis recovered, tested negative, and returned to work. As a result of his experience and his interest in medicine, Luis has done a lot of research on the vaccine. Luis is bilingual and volunteers with CARE Chest of Sierra Nevada, a non-profit organization that provides free medical equipment, supplies, and prescription assistance to underserved populations when he is not working at Renown. Vaccine one more tool to help fight pandemic “The best protection from COVID-19 will be a combination of getting a COVID-19 vaccine, wearing a mask, staying at least 6 feet away from others, avoiding crowds, and washing your hands often,” says Amy McCombs, MSN, RN, CCRN, CNML, vice president of Quality for Acute Services for Renown, and the champion of the vaccination effort for Renown. “No one tool alone will stop this pandemic, it will take a combination of all of us using all of these efforts to fight the spread of this virus. Though there has been positive news about vaccine development, no one knows when those vaccines will be ready for widespread use. In the meantime, everyone must remain vigilant, take precautions and follow public health orders.” “This vaccination is a game-changer in terms of improving the health of our community,” said Paul Sierzenski, MD, MSHQS, Chief Medical Officer, Acute Services, Renown Health. "Given the demands of this COVID-19 pandemic, we are pleased to be able to provide this vaccination to help in the fight. Generic Drugs – What You Need to Know About Them
Without a doubt, taking medications can not only be expensive, but also confusing. In the United States, generic prescriptions are widely used, with 9 out of 10 people choosing them over a name brand. Pharmacists are a great resource to help us understand the benefits and side effects of any medication. We asked Adam Porath, PharmD, Vice President of Pharmacy at Renown Health, to answer some common questions about generic drugs. What is a generic drug? A generic drug has the same active ingredients of brand-name drugs. Brand-name drugs have a patent (special license) protecting them from competition to help the drug company recover research and development costs. When the patent expires other manufacturers are able to seek approval for a generic drug. However, the color, shape and inactive elements may be different. Per the U.S. Food & Drug Administration (FDA), a generic medicine works in the same way and provides the same clinical benefit as its brand-name version. Why do they cost less? Generic drug makers do not have the expense of costly development, research, animal and human clinical trials, marketing and advertising. This savings is passed on to the public. Also after a patent expires, several companies will compete on a generic version of a drug, further driving down prices.

We fill prescriptions, enter prescriptions into our system, receive and send orders for medications and maintain a clean pharmacy with an accurate inventory.” “We do a variety of things; the task people probably know the most is counting out the medications and putting them in the amber vials,” added Rachel Vallin, Pharmacy Technician in the Renown Regional Outpatient Pharmacy. “We also help patients at the front of the pharmacy, ring out their prescriptions, answer some basic questions (deferring to a pharmacist as necessary) and billing insurance. Meds to Beds is my favorite part because I feel the most involved. I take medications to patients who are discharging up to their hospital rooms so they have it with them when they leave.” “As a technician, I confirm that all medications of new admissions are available in our machines prior to admitting and then maintain stock during each patient’s stay,” added Tammara Axtman, Pharmacy Technician at Renown Rehabilitation Hospital. "I also assist our nurses when needed in regard to any of their questions with both EPIC and Omnicell.” Our pharmacy team is also on the move all across our health system, thanks to our Ambulatory Pharmacy programs. For patients experiencing a serious heart, lung, or endocrine condition that requires ongoing drug therapy maintenance and guidance, our ambulatory pharmacies step in to carefully monitor how their medications impact their health and well-being. “Our role as pharmacists in this department is non-traditional because we actually see patients in the exam rooms face-to-face,” said Cory Lankford, Ambulatory Care Clinical Pharmacist for Renown’s Anticoagulation Services. “We modify their medication regimens and drug recommendations under collaborative practice agreements.” “Because our role is so unique, we have a lot of opportunities to make a positive impact on patients,” added Janeen Abe, Ambulatory Care Clinical Pharmacist for Renown’s Anticoagulation Services. “We do a lot of direct patient interaction, including counseling patients on their medications and helping them navigate through their disease state.” “As a medical assistant in this department, we’re called the patient ‘liaisons’ to orchestrate who they should go to whether it’s a nurse, a provider or a pharmacist,” added Kiara Scruggs, Medical Assistant for Renown’s Anticoagulation Services. “We look at each patient’s medications and help with the Warfarin blood thinner monitor. We get to do a lot with patients." A key resource within the Pharmacy department and the emergency admission process, our Medical Reconciliation ("Med Rec") team stays on top of each patient's medication records. By ensuring each medication regimen is accurately reflected in each patient's chart and that patients continue to take their at-home medications while admitted to the hospital, this team provides vital insight into medications that could be a contributing factor to each patient's symptoms, including drug interactions. “Our medication reconciliation pharmacy technician team are true detectives,” said Heather Townsend, Clinical Pharmacy Supervisor. “When a patient arrives to the hospital, Med Rec works with patients, families, caregivers and outpatient pharmacies to compile a list of medications the patient has been taking a home. This list is used to make sure medications are not contributing to the patient’s symptoms and to assure medications are continued throughout the hospital stay. The addition of the medication reconciliation team has been one of the greatest advancements in medication safety.” “As a Med Rec Tech, we interview patients and family members and call pharmacies, skilled nursing facilities, etc. to obtain an accurate and complete medication list/history to outline what the patient is currently taking on a daily basis,” added Kara McGee, Medical Reconciliation Pharmacy Technician. “We make sure that we document the correct medication, dose, route, frequency and directions. Renown's Dr. Kate Ward Honored with Nevada Society of Health System Pharmacists (NVSHP) President's Award
The award is given annually to a member of the Nevada pharmacy community making significant and sustained contributions to the field Kate Ward, Pharm.D., BCPS Director of Clinical Pharmacy at Renown Health is the 2022 recipient of the Nevada Society of Health System Pharmacists (NVSHP) President’s Award. Dr. Ward has been with Renown for over a decade, starting as a Clinical Pharmacist in 2009. As the Director of Clinical Pharmacy, she is responsible for the oversight of all clinical pharmacy activities at Renown Regional, South Meadows and Rehab. Additionally, Dr. Ward is the Residency Director of the Post-Graduate Year 1 (PGY1) Pharmacy Practice Residency at Renown Regional Medical Center. “We are so proud that Kate has been recognized by the Nevada Society of Health System Pharmacists with the President’s Award and for her many accomplishments as a member of our Renown Pharmacy team. With 66% of adults taking five or more drugs per day and 27% taking 10 or more per day, a pharmacist’s review is critical in supporting patient safety and well-being,” says Tom Graf, MD, interim CEO, Renown Health. “Beyond that, pharmacists have the ability to suggest methods for accurately and effectively taking prescribed medications, discuss medication safety, identify and manage side effects and assist patients and families in managing chronic health conditions.” As a licensed pharmacist, Dr. Ward holds The Doctor of Pharmacy degree (abbreviated Pharm.D. or PharmD) from the University of Colorado. The Pharm.D. is a professional degree similar to a Doctor of Medicine (MD) or Doctor of Dental Surgery (DDS). As a doctorate, it represents the increasing responsibility pharmacists have in healthcare systems and the high trust Americans have in pharmacists. The Doctor of Pharmacy degree takes three to four years of undergraduate pre-professional (prerequisite) coursework, followed by four academic years in a professional program. Most students need four years to complete their prerequisite courses. Thus, it usually takes eight years of college study to earn a Pharm.D. and become a pharmacist. Pharmacists working in hospitals and health systems often complete additional post-graduate training after a Pharm.D. degree. Dr. Ward completed a PGY1 Residency at Stanford prior to joining the Pharmacy Department at Renown. “Dr. Ward is such an asset to Renown and our community. Her incredible perseverance to reach for and achieve higher aspirations has resulted in this important recognition by our peers across the state. She consistently contributes her pharmacy expertise to patients, physicians, clinicians and healthcare students and volunteers her time to advance the profession at both the state and national level through pharmacy professional organization participation. On Sept. 24, NVSHP and the Nevada Pharmacy Alliance held their annual joint meeting at Renown Regional Medical Center. During this meeting, Dr. Ward was awarded the NVSHP President’s Award. Dr. Ward was specifically recognized for representing the State of Nevada at the American Society Health System Pharmacists House of Delegates for several consecutive years, as well as her continuous contributions to the annual NVSHP Student Symposium. Established in October 2006, NVSHP has diligently worked on continuing the strong foundation developed to better meet the needs of health-system pharmacists in Nevada. Final Doses of COVID-19 Vaccines Administered to Renown Physicians
Renown Health is working with the Washoe County Health District (WCHD), the state, and the Centers for Disease Control and Prevention (CDC) to distribute the initially available, limited supplies of COVID-19 vaccines. Because there may be limited supply, the vaccine continues to be distributed in a tiered approach. Today, the light at the end of the tunnel shined brighter than ever as many of Renown’s Tier 1A recipients received their second and final doses. An important pandemic milestone “The health and safety of our staff is our top priority, and we remain steadfast in our determination to fight the good fight for our community. Administration of the final doses of the vaccine is an important pandemic milestone as it brings an overwhelming sense of hope and relief to all of us at Renown,” said Tony Slonim, MD, DrPH, President & CEO of Renown Health. “The healthcare heroes vaccinated today have been batting this pandemic on the front lines and working around the clock for the past 11 months to keep our community safe. It’s reassuring knowing they have an added, strong layer of protection, so they can focus their efforts on putting an end to this long and difficult battle.” Today, Renown healthcare workers received the second dose of the Pfizer-BioNTech vaccine which received emergency use authorization from the Food and Drug Administration last month. Administered 21 days after the initial dose, this dose marks the final dose of the vaccination series. Many Renown employees have also received the Moderna vaccine, of which second doses will be administered after 28 days. The intricacies of vaccine administration Renown’s COVID-19 vaccination process includes pre-scheduling physicians and employees through the Epic MyChart electronic medical record used by Renown. Employees are masked, and in their personal vehicles, as they drive through the vaccination site at Hometown Health. Each physician and healthcare worker provides their verification, identification, documentation and consent. They then extend their upper arm to receive the vaccination. Following the receipt of the vaccination, they have a 15-minute observation period to assure they do not experience any immediate side effects. Similar to receiving the first dose, these Tier 1A healthcare employees were encouraged to schedule their vaccination after their completed work schedule to have time to rest. As with all vaccinations, there is a potential for injection site reactions (redness, swelling and pain) as well as fever, fatigue, headache, chills, vomiting, diarrhea, muscle pain and/or joint pain. Fever is not an infrequent side effect, especially on the second dose, so these vaccinated employees will self-monitor their temperatures, as they cannot work at Renown with a fever. Renown invited media to help commemorate this triumphant vaccination event. Present during today’s event were- Dr. Bret Frey, representing the Medical Staff and Emergency Medicine physicians at Renown; Dr. Jacob Keeperman, Medical Director, Renown Regional Transfer and Operations Center representing physicians in Renown’s Intensive Care Unit; and Holly Irwin, a Clinical Respiratory Therapist. Vaccine one more tool to help fight pandemic “The best protection from COVID-19 will be a combination of getting a COVID-19 vaccine, wearing a mask, staying at least 6 feet away from others, avoiding crowds, and washing your hands often,” says Amy McCombs, MSN, RN, CCRN, CNML, vice president of Quality for Acute Services for Renown, and the champion of the vaccination effort for Renown. “No one tool alone will stop this pandemic, it will take a combination of all of us using all of these efforts to fight the spread of this virus. We are getting closer to widespread vaccine development every day; however, until then, everyone must remain vigilant, take precautions and follow public health orders.” “This vaccination is a game-changer in terms of improving the health of our community,” said Paul Sierzenski, MD, MSHQS, Chief Medical Officer, Acute Services, Renown Health. "Given the demands of this COVID-19 pandemic, we are pleased to be able to provide second doses of this vaccination to help in the fight. We continue to follow CDC recommendations for a tiered approach to vaccine distribution. Our goal is that by spring, all Renown physicians; employees and their family members; and all Hometown Health members and Renown patients interested in receiving the vaccine will have an opportunity to be vaccinated.” The COVID-19 vaccine is not mandatory for all Renown employees, but is strongly advised.
