Extracorporeal Membrane Oxygenation technology (ECMO) for patients with severe heart or respiratory failure is now available locally thanks to a grant from The Leona M. and Harry B. Helmsley Charitable Trust  Renown Health, the Level II Trauma Center for the region and the leader in offering the most innovative treatments for critically ill adults and children, is now home to northern Nevada’s first and only comprehensive extracorporeal membrane oxygenation (ECMO) program.  The ECMO machines arrived at the Cardiac ICU at Renown Regional Medical Center this week, and the multidisciplinary team of highly specialized physicians, Critical Care intensivists, Cardiovascular surgeons, perfusionists, registered nurses and therapists are ready to provide minute-by-minute support for patients receiving ECMO treatment. The multidisciplinary ECMO team includes pharmacists and respiratory therapists who are ready to receive their first patient. “At Renown Health, we are constantly investing in our clinicians, essential services, and medical innovations to help keep care local. Access to health care—both preventive care and advanced treatment—is critical for a community’s well-being and optimal health,” said Brian Erling, MD, MBA, President & CEO for Renown Health. “We are incredibly grateful for the Helmsley Charitable Trust and their $3.5-million grant to help establish the first ECMO program for the region at Renown. ECMO saves lives. Neighbors in need of ECMO life-support can now stay close to home and loved ones, and no longer need to be transported to facilities outside our area for this type of advanced care.” What is ECMO? “ECMO is a life support technology for patients with the most severe heart and lung illnesses,” explains Richard King, MD, FACEP Division Chief- Critical Care Medicine at Renown Health and Co-Medical Director of the ECMO program. “At Renown, our technology allows a patient's heart and lungs to heal and recover from illnesses, diseases, and injuries the body has sustained. The ECMO machine is a portable life-support system used for severe cases of heart and lungfailure. The ECMO procedure provides minimally invasive heart-lung bypass support outside of the body. The machine then removes carbon dioxide from the patient’s blood and adds oxygen. Basically, ECMO serves as your heart and lungs, supplying blood with oxygen and pumped back into the body, allowing your own organs to rest and to help the impaired function.” Why is ECMO important in saving lives?  “This state-of-the-art procedure can save lives in cases that otherwise would offer little chance of survival and can act as a bridge to those needing heart or lung transplants,” says Dr. Kimball Knackstedt, Cardiovascular surgeon, Renown Institute for Heart & Vascular Health, and Co-Medical Director of the ECMO program. “ECMO can be a game changer and a last resort for patients with severe heart and lung conditions. The procedure isn’t for everyone. ECMO requires anticoagulation, so a common complication is bleeding. This makes the therapy complex for trauma patients who often have life threatening heart and lung failure but due to severe bleeding, cannot utilize ECMO. The Helmsley Charitable Trust grant to the Renown Health Foundation will help give severe respiratory distress and cardiogenic shock patients a greater chance of survival and lessen the need for risky and lengthy transfers out of the community,” said Taralynn Bassham, BSN, Director of Nursing, Critical Care. “This investment in critical life support technology enables the best chance of survival for our patients suffering severe heart and/or lung failure during their most critical moments. This is truly a gift of life for many.”  ECMO programs growing across the U.S.  ECMO technology was developed in the late 1960s by a team led by Robert H. Bartlett, MD. Its main purpose was to serve as a long-term “bypass machine” to support the lungs or heart, which was not feasible at that time with conventional bypass machines. ECMO was first used successfully in 1971 by a patient with severe lung dysfunction after a motorcycle accident. In 1976, it was also used successfully for a baby who had severe lung injury from meconium aspiration, which led to increased use in the pediatric population. In 2009, ECMO was used worldwide in the treatment of severe lung failure during the H1N1 influenza pandemic. More than 100 new ECMO programs were launched in the U.S. in the last 3-5 years. Today, there are 6,120 hospitals in the U.S. and only 300 to 400 with ECMO programs according to the American Medical Association.  “We are launching our ECMO program with the capability to support up to 2 patients simultaneously for the first 1-2 years and aim to eventually perform 40 cases annually,” said Rob Blamire, Enterprise Project Manager for Renown. “Initially, we will focus on adult patients and later expand to include pediatric care. Depending on a patient’s needs, ECMO support varies from several days to weeks. There are several conditions extracorporeal life support can aid in treating, such as: Adult respiratory distress syndrome (ARDS); Burns (smoke inhalation); Cardiogenic shock; Heart attack; Respiratory failure; Myocarditis; Pulmonary embolism;Sepsis; and as a “bridge” to transplant. Clinical researchers with the Healthy Nevada Project co-author research paper with findings that emphasize the need for a comprehensive approach to breast cancer risk assessment – including a focus on genetic medicine – to help ensure that individuals at high risk are identified and supported proactively rather than reactively.  Breast cancer is a leading cause of cancer death among women in the United States. According to the American Cancer Society, about 1 in 8 women will develop breast cancer and about 1 in 39 women will die from breast cancer. Breast cancer is associated with increased age, hereditary factors, obesity, and alcohol use. Since 1990, breast cancer death rates have declined progressively due to advancements in treatment and detection. In Nevada there are an estimated 2,310 new breast cancer cases a year, and genetic mutations such as in the genes BRCA1 or BRCA2 remain a top risk factor for this prevalent disease. Recognizing the urgency for progress in breast cancer research, a collaborative effort between physicians, advanced practice providers and scientists from the Healthy Nevada Project® (HNP) and Helix have unveiled groundbreaking research. This study explores how genetic screenings are a necessary supplement to traditional testing methods, together offering more accurate insights into a patient's likelihood of developing breast cancer in the future. HNP is operated by Renown Genomic Medicine and the Institute for Health Innovation and is one of the largest community-based population health studies in the country. Their team works in collaboration with Helix, a leader in precision health that delivers comprehensive genomic solutions. Together, this dynamic partnership aims to understand breast cancer risk factors and pave the way for more effective preventative measures. The combined research team studied 25,591 female HNP participants to evaluate the performance of different genetic screening approaches to identify women at high risk of breast cancer. The results of this research suggest that a combined monogenic, or single-gene, and polygenic, or multi-gene, approach to breast cancer screenings helped produce more accurate results and more closely identify study participants who have a high genetic risk of developing the disease. "Based on this research, we are advocating a shift in approach which would improve breast cancer risk assessment through a combination of effective family history ascertainment and genetic screening,” said Joseph Grzymski, PhD, principal investigator of the Healthy Nevada Project, research professor at the University of Nevada, Reno School of Medicine and co-author of the breast cancer research paper. “This tailored approach, founded on the assessment of individual genetic risk, not only intends to elevate patient well-being but also will improve efficiency and equity in healthcare." Complementing the team’s research on leveraging genetics to identify women at low genetic risk of breast cancer that could safely defer mammogram screenings by five to 10 years that was released in late 2023 in JAMA Oncology, the study suggests that incorporating genetic information can assist in personalizing breast cancer screenings and optimizing the use of screening resources. "Existing disparities persist across various facets of breast cancer screening and treatment; however, genetic screening is clearly a powerful tool to help facilitate early intervention for those at higher risk,” said Jamie Schnell Blitstein, APRN, a primary care nurse practitioner at Renown Health and co-author of the breast cancer research paper. “By placing a heightened focus on risk, we underscore the pivotal role of preventative breast cancer screening.” Despite the availability of effective methods for early screening, co-authors of this research found that 78 percent of women with a family history of breast cancer had their risk ascertained only after a breast cancer diagnosis. The findings emphasize the need for a comprehensive approach to breast cancer risk assessment – including a focus on genetic medicine – to help ensure that individuals at high risk are identified and supported proactively rather than reactively.  “These findings that can profoundly impact how healthcare is delivered were only made possible by all the participants who were willing to consent to research,” said Alex Bolze, PhD from Helix and co-author of the publication. “Broad-scale collaboration projects like these between Renown Health and UNR that engage large populations where participants share both their genetic information as well as electronic health records drive advancements in preventative medicine, as well as fundamental biological research.”   The research paper was officially accepted on Jan. 29, 2024, and will be published by Elsevier, Inc. on behalf of the American College of Medical Genetics and Genomics. The Renown Institute for Health Innovation (IHI) and the University of Nevada, Reno School of Medicine (UNR Med) are excited to offer a free Enhanced Liver Fibrosis (ELFTM) test to patients at risk for a common type of liver disease, e.g. metabolic and nonalcoholic steatohepatitis (M/NASH), and who enroll or are currently enrolled in the Healthy Nevada Project. The ELF Test is an FDA (Food and Drug Administration) approved non-invasive test to help identify people most at risk for liver scarring, e.g. cirrhosis, and allows doctors to intervene before irreversible damage occurs. If undetected and untreated, M/NASH can result in liver cirrhosis which could require liver transplantation or lead to death. This test is important as the symptoms of M/NASH can be silent or non-specific, making it difficult to diagnose.  “There are currently more than 11,000 people across the U.S. on the national liver transplant waiting list, and with the heightened prevalence of NAFLD and M/NASH, this number is projected to keep rising; however, with research such as the ELF Test at our disposal, we are continuing to find ways to improve the health of those at risk for advanced liver disease,” said Bill Plauth, MD, Chief Medical Officer for Renown Health and Associate Dean of Clinical Affairs at UNR Med. “We encourage our community to participate in this early risk detection. Dr. Erling sent the following letter and this video to Renown’s employees and care providers. We are pleased to share it with our community. Hello! Today marks my first day as your Renown Health President and CEO, so I thought I would kick off the occasion with a note. Throughout the interview process, I have been beyond impressed with the passion and purpose demonstrated by all who I have met, and I am overwhelmed with gratitude for the warm welcome that we have received. I look forward to meeting each and every one of you, but it may take some time to get to all 7000 of you, so I thought I would share some information not included in the initial announcement. A bit about where I come from and what I believe, which informs how I approach leadership. Healthcare has been my passion for as long as I can remember. I grew up in Minnesota and started my healthcare career as a nursing assistant during high school, and then as a monitor tech during college. After medical school and training, I went on to spend the next dozen years in the ED of a Level I trauma center in west Denver. I met my wife, Jessica, while attending medical school in Baltimore. She was working as a nurse in the surgical ICU and I was smitten. I offered to help her with a 3 am turn and bed linen change for our trauma patient, and the rest was history. We have three kids ages 17-22. I may wear a suit, but first and foremost I am a husband, father, clinician and a caregiver. Over the last 20+ years, I have worked my fair share of nights, weekends, and holidays, and I know the sacrifice you and your families make to care for our community. I will never take it for granted. I have been the sole provider covering a busy night shift, and I know that the trust and collaboration with your immediate team matters more than anything I am putting in this email. In fact, being part of a high performing, mission-driven team is my number one motivation. It’s what gets me up in the morning and why I work in healthcare. I have provided clinical leadership for a large, regional health system and understand that quality, safety, and innovation comes from the people doing the work, not from a corporate office. But you also need strong managers and leaders to facilitate and prioritize the work, and to provide support and inspiration. I view our greatest resource to be our people (each of you). Fancy robots, imaging equipment, etc. are essential, but they are not why our patients choose us. They come for your brand of hometown, trusted care. I am very excited about the new partnership with Renown and the University of Nevada, Reno School of Medicine. While still in its infancy, it provides the platform for us to be the center for training, research, and tertiary care for the region (and beyond). Most health systems would love to have their own health plan. The reality is that you cannot meaningfully improve the health of the population without managing value and risk. We are fortunate to have Hometown Health as part of Renown Health, and in our community, and I am excited to work with them to fulfill the mission of the organization. I have worked in for-profit and not-for profit healthcare. Renown is unique in the market as the only nonprofit health system in the market. What that means is that we invest every penny earned back into caring for our community. We don’t send money to shareholders in California or Pennsylvania, we invest it right here in Nevada. That is the kind of organization that I want to work for and where I want my family to get care. Some of you may have noticed that healthcare leaders have an impressively short half-life. I recall not being terribly impressed with one of our hospital leaders early in my career, and one of my senior partners told me to “just wait, he’ll be gone in a year or two.” She was correct. In fact, I had five CEOs in the time I was practicing in Denver. It’s difficult to create and move an organization toward a unified vision when your leader changes every few years. As my track record will support, this is not my style. I have been in Colorado for over twenty years and now look forward to making Reno and Renown my home for years to come. We are in an unprecedented time in healthcare, as all U.S. health systems are experiencing skyrocketing expenses and insufficient revenue growth. Renown is not immune to these pressures, but we are well positioned in the market to weather the storm. By focusing on continuous clinical and operational improvement, Renown will continue to set the gold standard for community health system performance. Study shows that many household wells need better drinking water treatment and monitoring Outside of Nevada’s bustling cities, private wells are the primary source of drinking water, serving 182,000 people. Yet some of the tested private wells in Nevada are contaminated with levels of heavy metals that exceed federal, state or health-based guidelines, a new study published in Science of The Total Environment shows. Consuming water contaminated by metals such as arsenic can cause adverse health effects. Scientists from DRI and the University of Hawaii Cancer Center recruited households with private wells through the Healthy Nevada Project. Households were sent free water testing kits, and participants were notified of their water quality results and recommended actions they could take. More than 170 households participated in the research, with the majority from Northern Nevada around Reno, Carson City and Fallon. “The goals of the Healthy Nevada project are to understand how genetics, environment, social factors, and healthcare interact. We directly engaged our participants to better understand environmental contaminants that may cause adverse health outcomes,” said co-author Joseph Grzymski, PhD, research professor at DRI, principal investigator of the Healthy Nevada Project®, and chief scientific officer for Renown Health. Nearly one-quarter (22%) of the private wells sampled had arsenic that exceeded safe levels determined by the Environmental Protection Agency (EPA) — with levels 80 times higher than the limit in some cases. Elevated levels of uranium, lead, cadmium, and iron were also found. “We know from previous research that Nevada’s arid climate and geologic landscape produce these heavy metals in our groundwater,” says Monica Arienzo, PhD, an associate research professor at DRI who led the study. “It was important for us to reach out to community members with private wells to see how this is impacting the safety of their drinking water.” Fewer than half (41%) of the wells sampled used water treatment systems, and some treated water samples still contained arsenic levels over EPA guidelines. Although average levels of heavy metal contaminants were lower in treated water, many homes were unable to reduce contaminants to levels considered safe. The state leaves private well owners responsible for monitoring their own water quality, and well water testing helps ensure water is safe to drink. This study shows that more frequent testing is needed to ensure Nevada’s rural communities have safe drinking water. This is particularly important as the effects of climate change and population growth alter the chemistry of groundwater, potentially increasing metal concentrations. “The results emphasize the importance of regular water quality monitoring and treatment systems,” said co-author Daniel Saftner, MS, assistant research scientist at DRI. Although the research focused on wells in Nevada, other arid communities in Western states are facing similar risks of water contamination. Released in partnership with the Desert Research Institute: New research from the Healthy Nevada Project® finds that a confirmed diagnosis does not always result in changes to patient care. Presenting individuals with potentially life-altering health information doesn’t mean the individuals – or their healthcare providers – will act on it. Follow-up education and conversations about actionable care plans with patients and their doctors are key next steps, according to new research from the Healthy Nevada Project.  The Healthy Nevada Project is a genetic screening and research project that launched in 2016 as a partnership between DRI and Renown Health. The project now has more than 50,000 participants, with genetic sequencing provided by Helix.  Between September 2018 and September 2020, the Healthy Nevada Project successfully notified 293 participants that they were genetically at risk for hereditary breast and ovarian cancer syndrome, Lynch syndrome, or familial hypercholesterolemia – three common genetic conditions known collectively as the Centers for Disease Control and Prevention (CDC) Tier 1 conditions. In a study published today in Frontiers in Genetics, Healthy Nevada Project scientists looked at the impact that notifying a patient of a positive finding for a CDC Tier 1 condition had on the care that the patient received in the months and years that followed.  According to their results, among the 293 Healthy Nevada Project participants who were notified of their genetic risk of a CDC Tier 1 condition, 71 percent of participants with electronic health records shared their findings with healthcare providers. However, only 30 percent of the electronic health records for these patients contained documentation of the genetic diagnosis, and only 10 percent of examined patients experienced a possible change in care after receiving the results of their genetic screening.  “The Healthy Nevada Project was implemented with a ‘hands-off’ approach where the participants receive their findings and decide with whom and when to share those findings. The findings were not automatically added to their electronic health records,” said Dr. Gai Elhanan, health data scientist at DRI and co-lead author of the study. “What we’re learning now is that to ensure that important genetic findings are integrated into the care journey it is important to make their inclusion into the electronic health records part of the study.” This study builds on previous Healthy Nevada Project research published in Nature Medicine demonstrating the importance of screening for CDC Tier 1 conditions, which affect about one in 75 individuals and can be mitigated or even prevented from developing into disease when detected early. This study found that as many as 90 percent of the CDC Tier 1 cases are missed by clinical providers during normal clinical care screenings and examinations. During the current study, the Healthy Nevada Project scientists found that 19 percent of studied participants had already developed one of the CDC Tier 1 conditions, and thus would have potentially benefited from earlier notification about their condition. The study team hopes that their findings will encourage individuals in Nevada to obtain genetic testing for these relatively common conditions. Even if individuals are older or have already suffered from diseases related to these conditions, testing could also prove beneficial to siblings, children, and grandchildren who may also be at risk and who could subsequently be screened in the event of a positive finding. Experienced physician executive Dr. Thomas Graf named interim Today, the Renown Health Board of Trustees, led by Adam Kramer, announced that Dr. Tony Slonim’s tenure has ended as president and CEO of Renown Health. Thomas R. Graf, M.D., FAAFP will now serve as interim Chief Executive Officer. Dr. Graf will assume the title and authority of CEO in addition to his current role as Chief Clinical and Quality Officer at Renown, and Senior Associate Dean for Clinical Affairs for University of Nevada, Reno School of Medicine. Sy Johnson, MBA, Renown’s current Chief Operating Officer has been promoted to interim President and COO. Following the completion of a thorough investigation of concerns raised to the Board of Directors, the Board determined that Renown Health, its patients and staff required new leadership. Accordingly, Dr. Slonim has been terminated with cause as CEO of Renown Health. Mr. Kramer announced the transition today by writing, “a new direction and leadership is needed given the critical work ahead. I have complete faith in Dr. Tom Graf and Sy Johnson, and their senior leadership teams, who already oversee health care system operations, and will help ensure a smooth transition.” Dr. Graf will work in collaboration with Sy Johnson, MBA, interim President and Chief Operations Officer and an experienced team of healthcare executives, to lead Renown to achieve its strategic and operational goals.
