Renown Urgent Care and Emergency Room teams encourage you to stay safe. Urgent Care is available seven days a week at 10 locations across northern Nevada and Emergency Rooms 24/7.  Millions of people across the West live in areas where air pollution can cause serious health problems. Local air quality can affect our daily lives. Like the weather, it can change from day to day. Northern Nevada Public Health’s (NNPH) Air Quality Management Division (AQMD) has issued an Air Quality Advisory due to the Davis Fire burning in southern Washoe County. Air monitors for the most region hit the “Moderate” range Saturday night, which is 151-200 on the Air Quality Index (AQI). That means some members of the public may experience health effects. All residents in impacted areas are encouraged to regularly monitor the air quality nearest them at www.airnow.gov and limit outdoor exposure when they see or smell smoke. Exposure to elevated PM2.5 (fine particulate matter in smoke) concentrations can result in eye and throat irritation, headaches, nausea, shortness of breath, congestion, coughing, impaired lung function and chest pain, especially among sensitive individuals such as the elderly, children, people with asthma, people with heart or lung conditions, pregnant women and anyone who is exercising or working hard outdoors.   Aleise Whitlock, APRN, Section Chief for Renown Urgent Care advises that there are people more likely to be affected when fine particle pollution reaches an unhealthy level, including: People with asthma or breathing conditions like Chronic Obstructive Pulmonary Disease. People who have heart disease or high blood pressure. Children and older adults. People doing extended or heavy physical activity like playing sports or working outdoors. “Everyone should take precautions when the air quality is unhealthy. Air pollution can aggravate heart and cardiovascular disease as well as lung diseases like asthma and COPD. When the air quality is unhealthy, people with these conditions may experience symptoms like chest pain, shortness of breath, wheezing, coughing, or fatigue. If you are experiencing any of these symptoms, use your inhalers as directed and contact your health care provider,” says Ms. Whitlock. “If you do not have a health care provider, we are happy to see you. Renown Urgent Care offices are on almost every corner, with providers ready to see you.” She advises to take it easy and listen to your body, and: Limit, change, or postpone your physical activity level. Drink plenty of water.  Stay away from local sources of air pollution like busy roads and wood fires. Use a damp cloth or mask to reduce smoke inhalation if you must go outside. If you have asthma or other breathing conditions like COPD keep your relief/rescue inhaler with you. People with asthma should review and follow guidance in their written asthma action plan. Make an appointment to see your health provider to be sure you have an asthma action plan. When to Visit the Emergency Department Bret Frey, MD, President, Western Emergency Physicians and Renown Health Emergency Departments, says, “Seek emergency care if you or someone you know is experiencing any of the following symptoms due to smoke exposure: Severe shortness of breath or difficulty breathing-If breathing becomes labored or painful, you cannot catch your breath even after resting, this may be a sign of serious respiratory distress. Chest pain or tightness- Any discomfort in the chest, particularly if it is associated with breathing, should be evaluated immediately.  Persistent coughing or wheezing- If a cough doesn’t improve or worsens, or if you develop wheezing, you should seek emergency medical attention.  Confusion, dizziness or fainting- These can be signs of oxygen deprivation, and immediate care is critical. Bluish lips or face- A clear indicator that oxygen levels are dangerously low and emergency medical treatment is needed.  Burns or smoke inhalation from a fire- Individuals who have been exposed to fire, or smoke in a confined space, should be evaluated for potential lung damage and other complications.  “Even just one hour of smoke exposure can affect our lungs and hearts, and these consequences can reach people far beyond the direct area endangered by the fire. Children, older adults, pregnant women, people with preexisting cardiopulmonary disease, communities of color, economically disadvantaged communities, and people with preexisting chronic health conditions like asthma, diabetes, and obesity are all especially vulnerable to the air pollution that wildfires cause," says Dr. Frey. Burning Man is an annual event inspired by the values reflected in the Ten Principles and united in the pursuit of a more creative and connected existence in the world. This year, people from across the world are expected to make the journey in RVs and cars brimming with tents, food, water, artwork, and costumes, temporarily turning Black Rock into the state’s third largest city. “The Burning Man Project has done an excellent job preparing participants to have a safe, healthy, and wonderful time building Black Rock City and living on the playa for nine days to nurture the distinctive culture emerging from that experience. Located just off I-580, this location is designed to treat patients who need immediate attention for emergencies.  These locations are designed to assess and care for emergency situations, including: Heart attack symptoms Stroke symptoms Broken bones Shortness of breath Seizures Severe bleeding Head injuries Chest pains Severe pain Venomous stings and bite All of Renown’s emergency rooms are staffed by Northern Nevada Emergency Physicians, including pediatric specialists and physicians who have completed fellowship training in specialty areas such as sports medicine, ultrasound and emergency medical services. A few interesting medical conditions (Playa Foot and Playa Lung), are associated with Burning Man. The Black Rock Desert in northwestern Nevada is part of the lakebed of the ancient Lake Lahontan, a deep lake that existed 15,000 years ago. Lake Lahontan left fine sediments on the lake bottom to form the extremely flat surface. Playa dust is made of both larger and smaller particulate matter. The Playa dust is in the air, on the ground, and on everything in Black Rock City. People combat the dust with masks, bandannas, and colorful respirators to keep the chalky dust out of their mouths and nostrils, but the effort is often futile. Playa Foot is a malady unique to the Black Rock Desert caused by the alkali dust that makes up the desert. It is, in essence, a chemical burn. The best prevention is keeping feet covered as much as possible, applying a moisturizing skin lotion before bed and wearing socks and closed shoes each day on the playa. “Playa Lung, where the particulate is breathed into the lungs, may cause coughing, throat pain and loss of voice in the short-term,” says William Graham, MD, a specialist in Pulmonary Medicine and Critical Care Medicine at Renown Health. “The dust can cause health issues for smokers, children and elderly and people with pre-existing conditions like asthma and COPD. The best prevention is to get a mask that’s comfortable with n99 filters, wear the mask as much as possible and change the filters often. Be sure to drink lots of water and take protection from windstorms. A doctor visit is recommended if any effects of breathing in dust persist beyond a week or two.” Dr. Derek Beenfeldt is the Chief Medical Officer for Renown’s not-for-profit insurance arm Hometown Health and has been a frequent participant in the Burning Man experience over the years. In the interest of wanting every person to have a wonderful experience while here- especially since over half of the participants live outside the U.S. health care system, he reminds people of the number of free and low-cost on-site health care and testing resources provided on the playa at Black Rock City. “It is important for Burning Man participants to know that without US health care insurance coverage, the costs can be significant for transportation off the playa for a medical evacuation, hospital emergency department care or hospitalization. Even for people who have insurance, in the past, some have faced some significant out-of-pocket expenses for the flight or transportation to Reno-area hospitals or emergency departments.” He shares the following information; On Site Medical Response A community of EMTs, paramedics, nurses, medical students, and physicians are on site at Black Rock City in RVs to provide emergency response and urgent care to participants as needed. Local Hospital Response Since 2011, the Burning Man organization, has contracted with Humboldt General Hospital in Winnemucca, Nev., to staff and equip Rampart Urgent Care, a field hospital that offers X-rays, ultrasounds, a hand-held head scanner, lab tests and a pharmacy. The single-room wooden structure is easily seen- it is painted pink and yellow. The hospital’s 40 beds are located in adjoining tents. At Rampart, when possible, patients are treated on site. EMTs, medics and nurses are empowered to practice medicine to the fullest ability their licensing and credentialing allow. Patients are not charged for Rampart Urgent Care services, which is covered as part of their ticket fee. The Nevada State Health Division, Bureau of Health Protection Services is also on the playa. You will know them by the Nevada State Health Division logo on their shirts. All Renown’s physicians remind people that if they are experiencing symptoms of a heart attack or stroke, they should go to the closest emergency room (ER) to get needed medical care. Delaying care for a medical emergency, such as a heart attack or stroke, can be life threatening or lead to serious complications. Extracorporeal Membrane Oxygenation technology (ECMO) for patients with severe heart or respiratory failure is now available locally thanks to a grant from The Leona M. and Harry B. Helmsley Charitable Trust  Renown Health, the Level II Trauma Center for the region and the leader in offering the most innovative treatments for critically ill adults and children, is now home to northern Nevada’s first and only comprehensive extracorporeal membrane oxygenation (ECMO) program.  The ECMO machines arrived at the Cardiac ICU at Renown Regional Medical Center this week, and the multidisciplinary team of highly specialized physicians, Critical Care intensivists, Cardiovascular surgeons, perfusionists, registered nurses and therapists are ready to provide minute-by-minute support for patients receiving ECMO treatment. The multidisciplinary ECMO team includes pharmacists and respiratory therapists who are ready to receive their first patient. “At Renown Health, we are constantly investing in our clinicians, essential services, and medical innovations to help keep care local. Access to health care—both preventive care and advanced treatment—is critical for a community’s well-being and optimal health,” said Brian Erling, MD, MBA, President & CEO for Renown Health. “We are incredibly grateful for the Helmsley Charitable Trust and their $3.5-million grant to help establish the first ECMO program for the region at Renown. ECMO saves lives. Neighbors in need of ECMO life-support can now stay close to home and loved ones, and no longer need to be transported to facilities outside our area for this type of advanced care.” What is ECMO? “ECMO is a life support technology for patients with the most severe heart and lung illnesses,” explains Richard King, MD, FACEP Division Chief- Critical Care Medicine at Renown Health and Co-Medical Director of the ECMO program. “At Renown, our technology allows a patient's heart and lungs to heal and recover from illnesses, diseases, and injuries the body has sustained. The ECMO machine is a portable life-support system used for severe cases of heart and lungfailure. The ECMO procedure provides minimally invasive heart-lung bypass support outside of the body. The machine then removes carbon dioxide from the patient’s blood and adds oxygen. Basically, ECMO serves as your heart and lungs, supplying blood with oxygen and pumped back into the body, allowing your own organs to rest and to help the impaired function.” Why is ECMO important in saving lives?  “This state-of-the-art procedure can save lives in cases that otherwise would offer little chance of survival and can act as a bridge to those needing heart or lung transplants,” says Dr. Kimball Knackstedt, Cardiovascular surgeon, Renown Institute for Heart & Vascular Health, and Co-Medical Director of the ECMO program. “ECMO can be a game changer and a last resort for patients with severe heart and lung conditions. The procedure isn’t for everyone. ECMO requires anticoagulation, so a common complication is bleeding. This makes the therapy complex for trauma patients who often have life threatening heart and lung failure but due to severe bleeding, cannot utilize ECMO. We look forward to providing care for patients who are too unstable for transport and who could die before receiving needed care.” "Renown Health serves more than one million patients in a 100,000-square mile reach across Nevada, Lake Tahoe and eastern California. This is truly a gift of life for many.”  ECMO programs growing across the U.S.  ECMO technology was developed in the late 1960s by a team led by Robert H. Bartlett, MD. Its main purpose was to serve as a long-term “bypass machine” to support the lungs or heart, which was not feasible at that time with conventional bypass machines. ECMO was first used successfully in 1971 by a patient with severe lung dysfunction after a motorcycle accident. In 1976, it was also used successfully for a baby who had severe lung injury from meconium aspiration, which led to increased use in the pediatric population. In 2009, ECMO was used worldwide in the treatment of severe lung failure during the H1N1 influenza pandemic. More than 100 new ECMO programs were launched in the U.S. in the last 3-5 years. Today, there are 6,120 hospitals in the U.S. and only 300 to 400 with ECMO programs according to the American Medical Association.  “We are launching our ECMO program with the capability to support up to 2 patients simultaneously for the first 1-2 years and aim to eventually perform 40 cases annually,” said Rob Blamire, Enterprise Project Manager for Renown. “Initially, we will focus on adult patients and later expand to include pediatric care. Depending on a patient’s needs, ECMO support varies from several days to weeks. The entire Renown team takes immense pride in introducing this leading-edge life support technology to our community, benefiting area patients and their families.”  What conditions may ECMO support help treat? There are several conditions extracorporeal life support can aid in treating, such as: Adult respiratory distress syndrome (ARDS); Burns (smoke inhalation); Cardiogenic shock; Heart attack; Respiratory failure; Myocarditis; Pulmonary embolism;Sepsis; and as a “bridge” to transplant. Stage 2 Air Quality Index and alarming research studies from DRI, Renown Health and the Washoe County Health District leads to public health advisory- seek care if needed For people who suffer from asthma, wildfire smoke is more hazardous than other types of air pollution, according to a study from the Desert Research Institute (DRI), the Renown Institute for Health Innovation (Renown IHI) and the Washoe County Health District (WCHD). The study published in the journal Environmental Health examined associations between airborne particulate matter (PM) from sources such as wildfire, transportation and industry, and medical visits for asthma at Renown Health’s emergency departments and urgent care centers in Reno, Nev. during the six-year period from 2013-2018. In places like Reno, where wildfire events occur regularly during parts of the year and are expected to become more frequent in the future, an accurate understanding of the impacts of wildfire smoke on population health is critical. We have an exceptional process in place through our urgent care sites, Renown Medical Group offices and hospital emergency departments to screen, diagnose and treat members of our community who come to us for care related to asthma, chronic obstructive pulmonary disease (COPD), chronic bronchitis, emphysema, and other upper respiratory diseases that may be exacerbated by wildfire smoke. At the same time, we continue to develop and refine, in real-time, the data that supports our predictive analytic models. We are using every tool and resource-including scientific data and clinical reporting, to ensure that we are meeting both the health and healthcare needs of the people we serve. In the study, researchers found that air quality in the Reno area was affected by wildfire smoke on a total of 188 days during the study period. A total of 18,836 asthma-related emergency room and urgent care visits occurred over the same five-year period of time, indicating that the influences of wildfire smoke and other types of air pollution on this medical condition are important to understand. The Washoe County Health District – Air Quality Management Division (AQMD) has issued a Stage 2 Air Pollution Warning due to smoke from area wildfires. Air quality in the Reno-Sparks area is now “Very Unhealthy” with an Air Quality Index of 247. Renown Health is proud to offer same-day virtual visits to help community members maintain their health and address medical concerns amid the COVID-19 crisis and beyond. Virtual Visits are easily-accessible video conferencing visits with a health provider. They are used for primary care, specialty care and urgent care to enable patients to consult a care provider from the ease and comfort of their homes. “If there’s a bright spot in the midst of this pandemic, it’s that COVID-19 is pushing us to rapidly adopt new ways of thinking about and delivering healthcare–through new technologies,” said Dr. Tony Slonim, president and CEO of Renown. “Our team has worked diligently to make virtual visits available to our community as an added convenience for those who would prefer to stay in their homes. As we work to keep all of our patients safe and healthy, we want our community to know we’re here to serve any ongoing healthcare needs they have by expanding our care options.” Virtual Visits for Urgent Care For urgent care needs, individuals can log in to MyChart and request to “Talk to a Doctor” to receive care from a Renown provider. After filling in some information and confirming their healthcare concerns, Renown’s healthcare team will notify them by text or email when a provider is ready to talk to them virtually. Providers will help patients determine whether their health needs can be met from their home or if they need to seek in-person care. Virtual Visits for Primary Care and Specialty Care Many primary and specialty care appointments for adults and children are now available virtually through Renown Medical Group. Individuals with an existing Renown Medical Group provider are encouraged to call 775-982-5000 if they would prefer to see a provider from  home. Home Medical Visits Last week, Renown announced its partnership with DispatchHealth, which is now making house calls to individuals who prefer to be seen from their home by a licensed medical professional. This high-quality medical care option is available to all Reno/Sparks residents, both insured and uninsured. If a patient is uninsured, there is a flat fee of $275, which includes all medications administered and lab tests on site. “Whether a patient prefers to be treated at home or in one of our traditional office settings, we are prepared to provide the right level of care, at the right place, at the right time,” said Dr. Slonim. “From routine exams and checkups to illnesses and other conditions that may arise, it’s critical to continue seeing to your unique healthcare needs.” All virtual visits require a free MyChart account.
