Renown Uses Analytics and Predictive Modeling in Service to the Community Renown continues to use current local and national data, analytics and public data sets for predictive modeling in service to our community. When there is heightened concern around an illness, enhanced infection prevention strategies are in place to ensure Renown is ready with the appropriate supplies and alternate plans should the need arise. Renown medical experts report the following information this week: 1. Cases in Washoe County have started climbing. It appears the Omicron wave has started in northern Nevada. The good news is that studies continue to show that vaccines and previous infection still provide people with protection from severe disease. Predictive analytics at Renown show that the Omicron wave is expected to peak in northern Nevada sometime between January 10 and February 10. 2. Data continues to mount that Omicron infections are much less severe than previous variants. The Renown team is preparing to see an increase in hospitalized patients, but at a far lower rate than during previous surges. 3. Today, new patient hospital admissions to Renown Regional Medical Center and Renown South Meadows Medical Center for patients with COVID-19 are low. Today, of the 501 patients hospitalized at Renown, less than 9% or 43 patients are hospitalized with COVID-19. This may change at any time and medical experts and scientists at Renown will continue to monitor data on a daily basis. Renown Offers State-of-the-Art Remote Patient Monitoring to Enhance Patient-Centered Model of Care Renown continues to enact the emergency preparedness plans that have been in place since the beginning of the pandemic, creating additional capacity for inpatients while allowing Renown to continue to deliver high-quality care. Renown now has state-of-the-art clinical remote monitoring services to serve additional patients diagnosed with COVID-19 who are clinically stable or improving. Remote patient monitoring is now being visualized and monitored by clinical staff within the new Renown Transfer and Operations Center (RTOC). The Transfer and Operations Center is powered by Renown doctors, nurses, technicians and national technology partners help ensure seamless care for patients and providers. Through this highly coordinated care logistics system, Renown is able to customize health care to the needs of every patient; delivering the right care, at the right time and place across the integrated delivery system. The best protection continues to be getting a COVID-19 vaccine and booster, wearing a mask, staying at least 6 feet away from others, avoiding crowds, and washing your hands often. For the thousands of physicians and health care workers of Renown Health, this week is etched into the pandemic timeline as the start date of a nationwide wave that signaled the first vaccinations against COVID-19 and a beacon of hope for the future. A year ago, the nation’s health care systems in communities across the U.S. first began vaccinating physicians and health care workers caring for patients with COVID-19, and thousands received their first vaccine. One year ago, Renown Health, the Washoe County Health District (WCHD), the state and the Centers for Disease Control and Prevention (CDC) began to distribute the initially available, limited supplies of the COVID-19 vaccines in Washoe County. Media representatives viewed the first healthcare workers as they obtained their first dose of the life-saving vaccines. Luis Martinez, first health care worker vaccinated, now plans to attend medical school Luis Martinez, who served on the front-line of the pandemic at Renown Regional Medical Center, received the first of his Pfizer-BioNTech vaccine a year ago this week, after it received emergency use authorization from the Food and Drug Administration. He received the second and final vaccination in the series 21 days later. Luis has been at Renown since 2016, where he started as a phlebotomist and has been encouraged to advance his career in the medical field. He works as a technician on Renown’s Clinical Decision Unit — a short stay unit within the hospital for people who need clinical care, but do not need admittance to the hospital. Luis worked with patients and clinicians in the Clinical Decision Unit for a year before requesting to work in Renown’s Alternate Care Site, built in the Mill Street parking structure as a place to care for patients recovering from COVID-19 before they returned home. Today, Luis is working to fulfill his dreams of becoming a doctor. He took the Medical College Admission Test® earlier this year, interviewed virtually and applied to 24 medical schools. Luis has shadowed physicians in a number of medical fields, including internal medicine, emergency medicine, cardiology and orthopedics, and would be thrilled to train in any one of those areas. As for medical school, Luis was excited to hear about Renown’s new clinical affiliation with the University of Nevada, Reno School of Medicine (UNR Med). He is enthusiastic about becoming part of the next generation of Nevada's health care professionals, and his first choice is UNR Med. He expects an admission decision by UNR Med this winter. If that doesn’t work out, he says, he has an interview with leaders at the Kirk Kerkorian School of Medicine at UNLV in January. Luis is bilingual and understands that Nevada continues to face a shortage of physicians and affordable medical care. He volunteers with CARE Chest of Sierra Nevada, a non-profit organization that provides free medical equipment, supplies, and prescription assistance to underserved populations when he is not working at Renown. His dream is to graduate medical school and stay in the community to provide medical care as a physician. Temporary Deployable Medical Structure Outside Renown Regional Emergency Department On March 12, 2020, Renown erected a deployable medical facility to serve as a respiratory illness evaluation and screening center for Emergency Room patients at Renown Regional Medical Center. A similar tent was also set up outside the Emergency Room at South Meadows Medical Center. An additional layer of protection, this proactive measure helped to care for people with symptoms of respiratory illness, and to protect patients and staff in the Emergency Department and other areas of the hospital. The tents were dismantled on January 20, 2021. (See photo here.) Alternate Care Site at Mill Street Parking Structure at Renown Regional As many communities across the nation experienced an increase in COVID-19 cases, including significant surges in hospitalized patients, Renown’s leadership had the foresight to create an Alternate Care Site (ACS) in the Renown Regional Medical Center Mill Street Parking structure. The ACS served additional hospitalized patients and allowed caregivers to remain on campus and still have access to existing hospital infrastructure such as lab, pharmacy, imaging, food services and other critical services. This solution also supported an efficient level of care escalation and other emergent interventions as needed. After just 10 days of intricate construction, including flooring, electrical infrastructure, lighting, water, technology and wall partitions, the ACS was completed on April 3, 2020 with space to hold up to 1,400 patients. On Nov. 12, 2020, Renown opened the ACS to serve additional hospitalized patients diagnosed with COVID-19 who were clinically stable or improving. Healthcare workers at Renown cared for hundreds of patients in this site. In early Jan. 2021, the remaining patients returned home. The Site has been decommissioned and leaders expect the area to be available for additional parking later this winter. (Alternate Care Site B-Roll.) Renown invests in Remote Patient Monitoring (RPM) ‘Hospital at Home’ Monitoring Systems Appropriate patients (including those recovering from COVID-19) are now offered a telehealth solution using wearable, continuous pulse oximetry, and the convenience of being in their own homes, with the confidence and security of knowing that Renown clinical staff are closely monitoring their health data through the Renown Transfer and Operations Center, and regularly communicating with them through their recovery process. The Renown Transfer and Operations Center spans 6,000 square feet, and is one of the largest centers of its kind in the nation, equipped with 28 high-definition dashboards on the front wall, two in the center of the room and four hanging monitors toward the back. Large plasma screens display important metrics such as patient vital signs, bed availability, facility capacity, staffing, technology and more. Registered Nurses and dispatchers continuously monitor activity around the clock, across Renown’s 100,000 square mile service area, 100+ locations and partner facilities to make the best decisions for patients, providers and care teams. “LOVE” Makes its Way to Renown On April 16, 2020, during a time of darkness and uncertainty, Artown loaned the “LOVE” sculpture, a one-ton, welded-aluminum artwork created by artist Laura Kimpton and fabricated by Jeff Schomberg to Renown, where it lit up Regional Medical Center’s main entrance on Mill St. The sculpture formerly debuted at Burning Man in 2019. In addition, Renown has stretched to support the Washoe County Health District to meet the needs of those aged 70 and older who are now eligible to receive a COVID-19 vaccine.  Over this past weekend, more than 3,000 of Washoe County’s oldest residents were able to obtain an appointment, and receive their first dose of the COVID-19 vaccine at Renown’s Drive Thru COVID-19 vaccination clinic. Renown posted 3,000 appointments for those on the MyChart electronic waiting list and 100% of appointments were booked within an hour. “We want to help the Health District, and we are working to vaccinate as many people as possible, given our limited staffing, volunteers, space and vaccine,” says Melodie Osborn, RN, Chief Nursing & Quality Officer for Health Services. “We are glad to step in and help; and our goal is to always provide an exceptional vaccine experience. Our team and our systems are definitely feeling the pressure as we see unprecedented demand for this vaccine. We ask the public to be patient with us, and to help support us in this important work.” Scheduling limited Both the Pfizer and Moderna COVID-19 vaccines require two doses, the second administered three to four weeks after the first. Renown is working to be sure those who receive their first dose also obtain their second dose in a timely manner. This includes 7,000 physicians and health care workers, and will now include those older adults vaccinated with their first dose. The complexity of the vaccine dosage requirements continues to draw down on the number of vaccines and appointments available.  Renown MyChart and phone systems strained under the demand Last Thursday, when Renown announced that those who are 75 years or older could join the waiting list by downloading Renown EPIC MyChart, so many people logged in that the system was strained to accommodate the additional demand. Renown has now doubled the capacity of the EPIC MyChart system to be sure all goes smoothly.  Renown Health has been able to distribute and administer the COVID-19 vaccine to a record number of people in the shortest possible time. Much of this success is due to the use of EPIC MyChart, a convenient patient electronic medical record which allows people to schedule their appointments online, complete the consent form and have full documentation for the future. In addition, the information automatically posts to Web IZ Nevada’s Statewide Immunization Information System. Also last Thursday, older adults without cell phones, computers or someone to assist them, called Renown’s customer service phone line for assistance. So many people called the phone line that it crashed. Beginning Thursday, January 14, 2021, in order to quickly vaccinate additional residents of Washoe County age 75 and older, Renown Health is pleased to partner with the Washoe County Health District to help distribute the initially available, limited supplies of COVID-19 vaccines.  Those who are 75 years or older interested in joining the waiting list for an appointment at Renown’s Drive Thru COVID19 vaccination clinic, will need to download Renown MyChart, if they currently do not have an account. Residents do not need to have a Renown Health provider to enroll. Residents will be notified through their MyChart account when appointments are available. The vaccine clinic is operating by appointment only. Due to limited resources and the limited vaccine available, initially the Renown COVID-19 Vaccination Program will concentrate on vaccinating those 75 and older at this time in Washoe County. Per Governor Sisolak, and the state’s vaccine rollout plan, once those 75 and older have the opportunity to receive the vaccine, the next general population priority group is those who are 70 and older. In addition, Renown Health patients and members of Senior Care Plus/Hometown Health aged 75 and older are will be notified through their Renown MyChart accounts as appointments become available. Once an appointment is confirmed, instructions are given on how to obtain the vaccine. “We remain steadfast in our determination to fight the good fight for our community. The tragedy of COVID-19 is that the mortality rates, or the risk of dying, is highest in this generation,” said Tony Slonim, MD, DrPH, President & CEO of Renown Health. “We are pleased to be part of the solution, to bring hope and to make a genuine difference in the health and well-being of others.”  Age increases risk for severe illness & hospitalization According to the CDC, the risk for severe illness with COVID-19 increases with age, with older adults at highest risk. The greatest risk for severe illness from COVID-19 is among those aged 85 or older. Severe illness means that a person with COVID-19 may require hospitalization, intensive care, or a ventilator to help them breathe, or they may even die. In Washoe County alone, there are 29,034 people 75 years or older and Renown is only receiving limited supplies of the vaccine. Older adults are at greater risk of requiring hospitalization or dying if diagnosed with COVID-19. As you get older, your risk of being hospitalized for COVID-19 increases. For those aged 75 and older, the risk of hospitalization is eight to 13x higher, with deaths 220-630 percent higher than for younger adults. Compounding the risk for COVID-19 for older adults is a weakened immune system. “Because the immune system generally weakens with age, it’s important that older adults get a vaccine to help their bodies form a defense against COVID-19” says Derek Beenfeldt, MD, Chief Medical Officer for Hometown Health, the insurance division of Renown. “Please get vaccinated, unless you are sick or have significant allergic reactions to vaccines."  You are advised not to receive the vaccine if: You are ill at the time of vaccination (fever, shortness of breath, vomiting, diarrhea or acute illness). You have been diagnosed with COVID-19 in the previous 14 days. You have received any other vaccination in the last 14 days. You have a history of significant allergic reaction to this vaccine or any of its ingredients. Due to unprecedented demand, Renown is coordinating with local and state agencies to ensure every person has an opportunity to receive the vaccine over time. Renown Health is working with the Washoe County Health District (WCHD), the state, and the Centers for Disease Control and Prevention (CDC) to distribute the initially available, limited supplies of COVID-19 vaccines. Because there may be limited supply, the vaccine continues to be distributed in a tiered approach. Today, the light at the end of the tunnel shined brighter than ever as many of Renown’s Tier 1A recipients received their second and final doses.  Please see B-roll from today's event and photos, B-roll and interviews from the first event here. An important pandemic milestone “The health and safety of our staff is our top priority, and we remain steadfast in our determination to fight the good fight for our community. Administration of the final doses of the vaccine is an important pandemic milestone as it brings an overwhelming sense of hope and relief to all of us at Renown,” said Tony Slonim, MD, DrPH, President & CEO of Renown Health. “The healthcare heroes vaccinated today have been batting this pandemic on the front lines and working around the clock for the past 11 months to keep our community safe. It’s reassuring knowing they have an added, strong layer of protection, so they can focus their efforts on putting an end to this long and difficult battle.”  Today, Renown healthcare workers received the second dose of the Pfizer-BioNTech vaccine which received emergency use authorization from the Food and Drug Administration last month. Administered 21 days after the initial dose, this dose marks the final dose of the vaccination series. Many Renown employees have also received the Moderna vaccine, of which second doses will be administered after 28 days.  The intricacies of vaccine administration Renown’s COVID-19 vaccination process includes pre-scheduling physicians and employees through the EPIC MyChart electronic medical record used by Renown. Employees are masked, and in their personal vehicles, as they drive through the vaccination site at Hometown Health. Each physician and healthcare worker provides their verification, identification, documentation and consent. They then extend their upper arm to receive the vaccination. Following the receipt of the vaccination, they have a 15-minute observation period to assure they do not experience any immediate side effects.  Similar to receiving the first dose, these Tier 1A healthcare employees were encouraged to schedule their vaccination after their completed work schedule to have time to rest. As with all vaccinations, there is a potential for injection site reactions (redness, swelling and pain) as well as fever, fatigue, headache, chills, vomiting, diarrhea, muscle pain and/or joint pain. Fever is not an infrequent side effect, especially on the second dose, so these vaccinated employees will self-monitor their temperatures, as they cannot work at Renown with a fever. Renown invited media to help commemorate this triumphant vaccination event. Present during today’s event were- Dr. Bret Frey, representing the Medical Staff and Emergency Medicine physicians at Renown; Dr. Jacob Keeperman, Medical Director, Renown Regional Transfer and Operations Center representing physicians in Renown’s Intensive Care Unit; and Holly Irwin, a Clinical Respiratory Therapist. Vaccine one more tool to help fight pandemic “The best protection from COVID-19 will be a combination of getting a COVID-19 vaccine, wearing a mask, staying at least 6 feet away from others, avoiding crowds, and washing your hands often,” says Amy McCombs, MSN, RN, CCRN, CNML, vice president of Quality for Acute Services for Renown, and the champion of the vaccination effort for Renown. “No one tool alone will stop this pandemic, it will take a combination of all of us using all of these efforts to fight the spread of this virus. We are getting closer to widespread vaccine development every day; however, until then, everyone must remain vigilant, take precautions and follow public health orders.”  “This vaccination is a game-changer in terms of improving the health of our community,” said Paul Sierzenski, MD, MSHQS, Chief Medical Officer, Acute Services, Renown Health.
