Extracorporeal Membrane Oxygenation technology (ECMO) for patients with severe heart or respiratory failure is now available locally thanks to a grant from The Leona M. and Harry B. Helmsley Charitable Trust  Renown Health, the Level II Trauma Center for the region and the leader in offering the most innovative treatments for critically ill adults and children, is now home to northern Nevada’s first and only comprehensive extracorporeal membrane oxygenation (ECMO) program.  The ECMO machines arrived at the Cardiac ICU at Renown Regional Medical Center this week, and the multidisciplinary team of highly specialized physicians, Critical Care intensivists, Cardiovascular surgeons, perfusionists, registered nurses and therapists are ready to provide minute-by-minute support for patients receiving ECMO treatment. The multidisciplinary ECMO team includes pharmacists and respiratory therapists who are ready to receive their first patient. “At Renown Health, we are constantly investing in our clinicians, essential services, and medical innovations to help keep care local. Neighbors in need of ECMO life-support can now stay close to home and loved ones, and no longer need to be transported to facilities outside our area for this type of advanced care.” What is ECMO? “ECMO is a life support technology for patients with the most severe heart and lung illnesses,” explains Richard King, MD, FACEP Division Chief- Critical Care Medicine at Renown Health and Co-Medical Director of the ECMO program. “At Renown, our technology allows a patient's heart and lungs to heal and recover from illnesses, diseases, and injuries the body has sustained. The ECMO machine is a portable life-support system used for severe cases of heart and lungfailure. The ECMO procedure provides minimally invasive heart-lung bypass support outside of the body. The machine then removes carbon dioxide from the patient’s blood and adds oxygen. Basically, ECMO serves as your heart and lungs, supplying blood with oxygen and pumped back into the body, allowing your own organs to rest and to help the impaired function.” Why is ECMO important in saving lives?  “This state-of-the-art procedure can save lives in cases that otherwise would offer little chance of survival and can act as a bridge to those needing heart or lung transplants,” says Dr. Kimball Knackstedt, Cardiovascular surgeon, Renown Institute for Heart & Vascular Health, and Co-Medical Director of the ECMO program. “ECMO can be a game changer and a last resort for patients with severe heart and lung conditions. The procedure isn’t for everyone. ECMO requires anticoagulation, so a common complication is bleeding. This makes the therapy complex for trauma patients who often have life threatening heart and lung failure but due to severe bleeding, cannot utilize ECMO. This is truly a gift of life for many.”  ECMO programs growing across the U.S.  ECMO technology was developed in the late 1960s by a team led by Robert H. Bartlett, MD. Its main purpose was to serve as a long-term “bypass machine” to support the lungs or heart, which was not feasible at that time with conventional bypass machines. ECMO was first used successfully in 1971 by a patient with severe lung dysfunction after a motorcycle accident. In 1976, it was also used successfully for a baby who had severe lung injury from meconium aspiration, which led to increased use in the pediatric population. In 2009, ECMO was used worldwide in the treatment of severe lung failure during the H1N1 influenza pandemic. More than 100 new ECMO programs were launched in the U.S. in the last 3-5 years. Today, there are 6,120 hospitals in the U.S. and only 300 to 400 with ECMO programs according to the American Medical Association.  “We are launching our ECMO program with the capability to support up to 2 patients simultaneously for the first 1-2 years and aim to eventually perform 40 cases annually,” said Rob Blamire, Enterprise Project Manager for Renown. “Initially, we will focus on adult patients and later expand to include pediatric care. Depending on a patient’s needs, ECMO support varies from several days to weeks. There are several conditions extracorporeal life support can aid in treating, such as: Adult respiratory distress syndrome (ARDS); Burns (smoke inhalation); Cardiogenic shock; Heart attack; Respiratory failure; Myocarditis; Pulmonary embolism;Sepsis; and as a “bridge” to transplant. A coalition of 60 top hospitals and healthcare institutions have joined forces in a nationwide campaign to encourage adults to get vaccinated against COVID-19. Led by Cleveland Clinic and Mayo Clinic, the campaign "Get the Vaccine to Save Lives" is designed to reassure the public that vaccines are safe, effective and necessary to achieve herd immunity and return to normal activities. "We're asking people to talk to their healthcare providers if they have questions and then get vaccinated," says Gianrico Farrugia, M.D., Mayo Clinic's president and CEO. "The vaccine is our strongest asset to end the pandemic, and I urge everyone who is eligible to get whichever vaccine you're first offered to save lives." The campaign hopes to reach adults who are hesitant to receive a vaccine, including racial and minority ethnic groups and people living in rural communities. According to a Kaiser Family Foundation survey published March 30, 17% of the public say they will take a wait-and-see approach before getting it themselves, and another 20% say they will never get a vaccine or will only get it if required to do so for work, school or other activities. This leaves a significant portion of the population at risk of going unvaccinated.  The nationwide campaign includes print and digital advertising, media outreach, social media, an awareness video and an informational website. Patients who qualify for a “Food is Medicine Prescription” can receive a bag of food for themselves and each member of their family every week through this free assistance program. Renown Health is proud to offer supplementary food to patients in need through its food pantry. Established nearly a year ago, patients who identify as food- insecure are working with Renown’s community health workers to receive fresh produce, canned goods, non-perishable items and other pantry staples to help nourish themselves and their family at no cost. Thanks to the support of the Northern Nevada Food Bank and other local community partners, Renown’s food pantry is helping meet the food needs of homeless and low-income individuals in our community. “Patients’ responses to our food pantry has been amazing,” said Renown community health worker Seth Rubin. “They are extremely grateful when we give them a bag of food. Their eyes light up knowing that they will have enough food to eat for the week without having to worry about finding a source of food for their family.” To make this assistance program possible, Renown’s food pantry receives food deliveries every Wednesday from the Northern Nevada Food Bank. The Food Bank meets Renown’s team of community health workers, along with churches and other medical organizations, with a semi- truck loaded with a variety of foods. After receiving a pallet with their food order, Renown’s community health team collects an assortment of fresh produce, donated from local grocery stores and other community organizations. After that, the food is unloaded into Renown’s food pantry located inside The Healthcare Center. Renown’s community health workers serve patients in the hospital and emergency rooms, as well as individuals who receive care in The Healthcare Center, which provides affordable access to care for anyone in our community. After expressing a need for food assistance, patients are offered a Food is Medicine Prescription. Through this prescription, they can go to a participating food pantry once a week to receive one bag of food per person in their household. Pantry Continues Amidst Pandemic Since the beginning of the COVID-19 pandemic, Renown’s community health workers have worked remotely, but that hasn’t stopped them from reaching out to patients who benefit from this program. “We’re currently coordinating outpatient deliveries to our patients,” said Rubin. “Every Tuesday and Thursday, we deliver a bag of food to the patient’s house. It has been a great way to continue providing healthy food to our patients while continuing to provide safe care to the patients we see in person at our clinic. New study, led by physician researchers from Renown Health and University of Nevada, Reno School of Medicine seeks to understand how the immune system responds to COVID-19 with goal of developing a new treatment. Individuals who have recovered from COVID-19 may now be able to help patients currently fighting the infection by donating their plasma. Those who have recovered from the infection may have COVID-19 antibodies in their blood. These antibodies provided one way for their immune systems to fight the virus when they were sick, so their blood may be used to help others fight off the disease through convalescent plasma. Convalescent plasma is a component of blood from recovered patients that may contain precious COVID-19 antibodies. Antibodies are proteins that might help fight the infection. In this study, we will be collecting plasma from patients who have recovered from COVID-19 and investigating its efficacy in helping treat other patients with COVID-19. Convalescent plasma is being investigated for treatment of COVID-19 because there is no approved treatment for the disease and there is information that suggests it might help some patients recover from COVID-19. Renown Health and the University of Nevada, Reno School of Medicine (UNR Med) are leading a study locally to better understand how the body’s immune system responds to the virus, how it presents in northern Nevada and ultimately, aid in developing a new treatment for COVID-19. “Renown and UNR Med are at the forefront of conducting essential research to increase the health and safety of our community,” said Sara Healy, MD, MPH, principal investigator of the study and a pediatric infectious disease physician at Renown Children’s Hospital and UNR Med. “So little is known about effectively treating COVID-19 and we are venturing into new territory. There are 332 people, just in Washoe County (as of 5/1/20) who have recovered from COVID-19 and have immune systems that may now be producing antibodies to protect them from becoming infected again with coronavirus. Donated plasma is needed right now, for this clinical trial to determine definitively if this treatment works. Participating in this research study will also make it easier to donate plasma to the Mayo Clinic convalescent plasma program that Renown is a part of in hopes to find a treatment for COVID-19. There is no cost to participate in this study and participation is voluntary. An individual’s decision to participate will not affect their current or future relations with their health care provider(s), health district, or the community.
