Extracorporeal Membrane Oxygenation technology (ECMO) for patients with severe heart or respiratory failure is now available locally thanks to a grant from The Leona M. and Harry B. Helmsley Charitable Trust  Renown Health, the Level II Trauma Center for the region and the leader in offering the most innovative treatments for critically ill adults and children, is now home to northern Nevada’s first and only comprehensive extracorporeal membrane oxygenation (ECMO) program.  The ECMO machines arrived at the Cardiac ICU at Renown Regional Medical Center this week, and the multidisciplinary team of highly specialized physicians, Critical Care intensivists, Cardiovascular surgeons, perfusionists, registered nurses and therapists are ready to provide minute-by-minute support for patients receiving ECMO treatment. The multidisciplinary ECMO team includes pharmacists and respiratory therapists who are ready to receive their first patient. “At Renown Health, we are constantly investing in our clinicians, essential services, and medical innovations to help keep care local. Access to health care—both preventive care and advanced treatment—is critical for a community’s well-being and optimal health,” said Brian Erling, MD, MBA, President & CEO for Renown Health. “We are incredibly grateful for the Helmsley Charitable Trust and their $3.5-million grant to help establish the first ECMO program for the region at Renown. ECMO saves lives. Neighbors in need of ECMO life-support can now stay close to home and loved ones, and no longer need to be transported to facilities outside our area for this type of advanced care.” What is ECMO? “ECMO is a life support technology for patients with the most severe heart and lung illnesses,” explains Richard King, MD, FACEP Division Chief- Critical Care Medicine at Renown Health and Co-Medical Director of the ECMO program. “At Renown, our technology allows a patient's heart and lungs to heal and recover from illnesses, diseases, and injuries the body has sustained. The ECMO machine is a portable life-support system used for severe cases of heart and lungfailure. The ECMO procedure provides minimally invasive heart-lung bypass support outside of the body. The machine then removes carbon dioxide from the patient’s blood and adds oxygen. Basically, ECMO serves as your heart and lungs, supplying blood with oxygen and pumped back into the body, allowing your own organs to rest and to help the impaired function.” Why is ECMO important in saving lives?  “This state-of-the-art procedure can save lives in cases that otherwise would offer little chance of survival and can act as a bridge to those needing heart or lung transplants,” says Dr. Kimball Knackstedt, Cardiovascular surgeon, Renown Institute for Heart & Vascular Health, and Co-Medical Director of the ECMO program. “ECMO can be a game changer and a last resort for patients with severe heart and lung conditions. The procedure isn’t for everyone. ECMO requires anticoagulation, so a common complication is bleeding. This makes the therapy complex for trauma patients who often have life threatening heart and lung failure but due to severe bleeding, cannot utilize ECMO. This is truly a gift of life for many.”  ECMO programs growing across the U.S.  ECMO technology was developed in the late 1960s by a team led by Robert H. Bartlett, MD. Its main purpose was to serve as a long-term “bypass machine” to support the lungs or heart, which was not feasible at that time with conventional bypass machines. ECMO was first used successfully in 1971 by a patient with severe lung dysfunction after a motorcycle accident. In 1976, it was also used successfully for a baby who had severe lung injury from meconium aspiration, which led to increased use in the pediatric population. In 2009, ECMO was used worldwide in the treatment of severe lung failure during the H1N1 influenza pandemic. More than 100 new ECMO programs were launched in the U.S. in the last 3-5 years. Today, there are 6,120 hospitals in the U.S. and only 300 to 400 with ECMO programs according to the American Medical Association.  “We are launching our ECMO program with the capability to support up to 2 patients simultaneously for the first 1-2 years and aim to eventually perform 40 cases annually,” said Rob Blamire, Enterprise Project Manager for Renown. “Initially, we will focus on adult patients and later expand to include pediatric care. Depending on a patient’s needs, ECMO support varies from several days to weeks. There are several conditions extracorporeal life support can aid in treating, such as: Adult respiratory distress syndrome (ARDS); Burns (smoke inhalation); Cardiogenic shock; Heart attack; Respiratory failure; Myocarditis; Pulmonary embolism;Sepsis; and as a “bridge” to transplant. Two vascular surgeons bring a quarter of a century of experience to patients and community. Hansen, MD, FACS and Benjamin P. Jaquish, MD, FACS have joined Renown Health.  "Recognizing the significance of preventing, diagnosing and caring for people with heart and vascular disease, which continues to be on the rise and taking far too many lives, is of critical important to us at Renown," says Christos A. Galanopoulos, MD, MBA, MSc, FACS, VP, Chairman Department of Surgery at Renown Health. "Both Drs. Hansen and Jaquish are superb clinicians, skilled investigators, and effective educators. Together, the vascular surgeons have more than 26 years of combined experience caring for northern Nevadans. Dr. Hansen has 20 years of experience in northern Nevada. He is a clinical associate professor of surgery at the University of Nevada, Reno School of Medicine (UNR Med) and currently serves as medical director of vascular and endovascular surgery for Renown Regional Medical Center. "Since 2002, Dr. Hansen has led our region with his expertise and knowledge of aortic stents, grafting for abdominal aortic aneurysms and thoracic aneurysms, as well as treating patients with carotid disease," says Jayson Morgan, MD, FACC, Director, Cardiovascular Services. "Dr. Hansen specializes in the comprehensive management of vascular disorders, including open and endovascular surgeries. This gift will help establish the Helaine Greenberg Women’s Heart Center at Renown Health, located within the Heart & Vascular Institute. A first of its kind, the Center will offer specialized heart treatment for women in northern Nevada. Renown continues to have extra safety measures in place to protect the health of patients, visitors and healthcare employees, and as the situation changes with COVID-19, these guidelines are subject to change. Under this updated visitor policy, patients no longer need to identify their supporters ahead of their arrival. Patients may receive more than two visitors per day; however, to help ensure adequate physical distancing, only two guests are asked to visit at a time. All people entering Renown sites must continue bringing and wearing a face mask for their entire visit, including inside patient rooms. Renown’s visitor policy updates were inspired by family members, who requested that the policy be expanded to encourage two family members per day to be able to visit a loved one who was hospitalized at Renown. Renown hospitals are among the first in the country to lift visitor restrictions for patients with COVID-19 and encourage family to be at the patient's bedside. True treasure hunters, Dr. Dave and Darlene found joy in exploring antique shops and garage sales to find damaged or discarded vintage pieces. After hauling the item home, Dave would spend many hours scraping it clean, sanding and refinishing it, until it gleamed and became a polished, functional and beautiful piece of furniture. Every piece in their home rekindles a memory and has a story to tell.  On Thanksgiving, when Dave was too ill to gather around their antique dining room table, Darlene called the ambulance. Dave had been ill with COVID-19 for two weeks and had not been improving. When the EMTs reached her home and asked Darlene what underlying conditions he had, she said, “all of them.” David was seriously ill. Hospitalized for COVID-19, their communications options were limited. The only way Darlene could communicate with Dave was on a video call, or by telephone. Dave spent 17 days hospitalized at Renown Regional Medical Center in Reno. Darlene spent 17 days waiting by the phone for more information. Darlene said he had “up days and down days,” but thought he might be home, sitting at their antique dinner table for Christmas.  David Ivan Randolph was born in Boston, MA on August 29, 1936. He grew up in Roxbury and graduated from the prestigious Boston Latin Boys School. He obtained a Bachelor of Arts in General Psychology, an MA in Experimental Psychology from Northeastern University and a PhD in Sensory Psychology & Physiology from the University of Massachusetts, Amherst. While in university he joined ROTC and upon graduation was commissioned as a First Lieutenant in the Army Medical Specialist Corp. After basic training at the Fort Sam Houston Medical Training School he was assigned as a Captain, Research Psychologist to the US Army at the Pioneering Research Division in Natick, MA. Over the next 30 years, he conducted vision research at Fort Knox, KY; Philadelphia, PA and then with the newly constructed Letterman Army Institute of Research (LAIR), Division of Ocular Hazards at the Presidio of San Francisco, CA. David continued his vision research at LAIR until his retirement in 1992 when the Presidio was decommissioned. Dr. Randolph’s research findings in flash blindness and night vision made him a sought-after presenter at research conferences in the US and abroad. In a PBS documentary he was interviewed as part of the team that developed goggles that protect soldiers from laser eye damage. Dr. David Randolph lost his battle with COVID-19 on December 13, 2020, and died as he slept in a hospital bed. When Darlene wrote his obituary for the newspaper, she gave thanks to the “tremendous nurses and doctors at Renown Regional Medical Center, for providing his care during a time when family could not be with him.” Darlene wished she could have been there. Over their 45-year marriage, she had always been there. Darlene said, “I had always been at his bedside, as his advocate, to help communicate and straighten things out.” As a registered dietician, she worked in hospitals and knew the protocol. She knew, like hospitals across the globe, Renown had restricted family members from visiting in order to stop the spread of the virus- to other patients, staff and their family members. Still, she wished she could have spent more time with him. On Christmas Eve, she sat down and wrote to Dr. Anthony Slonim, president and chief executive officer of Renown Health in Reno, NV. “Dear Doctor Slonim, as the wife of a COVID patient who recently passed away in your hospital, I want to express my thanks to you and your staff for the care he was given in the last days of his life. I am aware that the nurses and staff are working under dangerous conditions and risking their health and lives by caring for multiple COVID patients. The staff was gracious, concerned and did everything they could for my husband and myself.” She continued, “I know procedures are changing every hour to try to stay ahead of this dangerous virus and I am sharing my experiences hoping they will be helpful when establishing policies that impact families.” Darlene explained that despite receiving assurances that Dave’s nurse or even a doctor would call daily, sometimes they would forget. She explained in her letter, “how important it was, in these times when family cannot visit, and has only infrequent communication and is anxiously waiting at home for word of their loved one, how much it means to get a call from someone caring for him at the hospital.” Darlene asked, “If there is any way you can help to assure that nurses have time to make calls or assist patients to make calls, because it is an important part of patient care.”  After receiving her letter, Dr. Slonim called Mrs. David Randolph, to thank her, to offer his sympathies and to ask if Renown could help in any way. Darlene asked if he might reconsider allowing families to visit hospital patients being treated for COVID-19. Dr. Slonim, who had also lost his father to COVID-19 back in April at a hospital in New Jersey, asked his leadership team to review Mrs. Randolph’s request and make a recommendation. The hospital leadership team made a recommendation to Dr. Slonim that was immediately approved. As the COVID-19 situation has evolved, the policy has as well. Adam D. Porath, Pharm.D., BCACP, BCPS-AQ Cardiology is promoted to serve future generations and improve the health and well-being of Nevadans. Renown Health is excited to announce Dr. Adam Porath has been promoted to vice president, pharmacy to lead clinical pharmacy and pharmacy operations and ensure excellent care for patients and families.  “We are pleased to promote an experienced and talented leader like Dr. Adam Porath,” said Dawn D Ahner, DSc, FACHE, Chief Operating Officer, Acute Services. “Adam will continue to foster a culture of caring and excellence, while ensuring the highest level of pharmacy practice and experience for both care providers and patients.”  Dr. Porath has served in roles within pharmacy at Renown since 2006. Since June 2020, he has served as Interim Director of Pharmacy where he has led strategic planning for pharmacy operations, precepted students and residents, identified business growth opportunities, developed novel clinical pharmacy services and oversaw quality assurance of pharmacy operations. He began his career as a pharmacy practice resident at Renown Regional Medical Center in Reno.  “I am honored to be given the opportunity to serve the medical community and patients of Renown Health,” said Dr. Porath. “What a source of pride it is to be able to deliver the high-quality, compassionate patient care Renown is known for, and to help serve and lead in the pharmacy profession.” Dr. Porath received his doctor of pharmacy degree from Idaho State University. He is a board certified pharmacotherapy specialist with added qualifications in cardiology and board certified ambulatory care pharmacist. Dr. Porath has a BS in Biology from the University of Nevada, Reno.  Dr. Porath serves as adjunct clinical faculty for several colleges of pharmacy including the University of New England, Oregon State University, University of New Mexico, Idaho State University and University of Wyoming. He has been published in several peer-reviewed journals including CHEST, Clinical Infectious Diseases, and the Canadian Medical Association Journal.  Here in Nevada, Dr. Porath successfully lobbied the Nevada Legislature on behalf of the Nevada Society of Health System Pharmacists (NVSHP) to pass Collaborative Drug Therapy Management (CDTM) legislation in 2011, permitting Nevada pharmacists to adjust drug therapy per physician-signed protocol in any practice setting, as long as the pharmacist is under a physician’s direct supervision. Currently, 48 states permit some degree of CDTM. In 2017, Adam helped to pass further expansion of the pharmacy practice act. He is currently serving as a director at large of the NVSHP board. Dr. Porath has served as a state of Nevada delegate to the American Society of Health System Pharmacists (ASHP) House of Delegates since 2012. In 2018, Dr. Porath was appointed to the Nevada Attorney General’s statewide partnership on the opioid crisis. In 2019, he was appointed to the ASHP Council on Public Policy.  In U.S. News and World Report Best Hospital rankings, Renown South Meadows Medical Center was listed #1 in the State of Nevada. Renown Health's Dr. Christopher Rowan recently joined a distinguished panel of experts on the "Game Changers in Medicine" podcast to discuss the science behind the discovery of warfarin. Renown Health Cardiologist Dr. Christopher Rowan was just featured on an episode of Game Changers in Medicine, a groundbreaking medical history podcast from Dramatic Health about some of the world’s most significant medical discoveries. The drug is especially important for individuals who have experienced a heart attack or stroke, or are at risk of experiencing either. However, the backstory of the drug is rather fascinating.  Beginning in the 1920s, an unknown disease was killing cattle throughout the U.S. and Canada. Desperate to find a solution, one farmer drove 200 miles to the University of Wisconsin with a dead cow, bales of spoiled hay and a milk can of unclotted blood in tow. He crossed paths with a scientist, whose team set out to determine the hemorrhagic component of the spoiled hay, resulting in a series of medical discoveries that would revolutionize medicine.  After learning more about Warfarin and how Vitamin K could reverse its blood thinning properties, doctors first prescribed the drug for human use in 1954. It is perhaps most widely known for saving President Dwight D. Eisenhower after he suffered a heart attack in 1955. Today, Warfarin continues to saves millions of lives each year. “Before blood thinners like Warfarin, people would experience strokes, blood clots in their lungs, blood clots in their legs, blood clots in their hearts, along with countless other clotting disorders - these conditions likely cutting their lifespans short,” Dr. Rowan said during the podcast. “Warfarin revolutionized the treatment of patients who suffer from heart attacks and strokes, helping them to live normal and healthy lives.
